Medical Oncology, Hematology Patient Population Mix (% of Peds, Adolescent, Adult, Geriatrics, Medicare, Medicaid, other): Adults, variety of payors Average Number of Cases per day (patients, inpatients, follow-ups, etc.): 14-20 follow up visits per day, no new consults Description of support staff: Full staff available – Reception, MA, Nursing, Chemo, Nurse Navigation plus non-clinical support available Locum Tenens work and call schedule: Clinic M-F, 8a – 5p, plus nights and weekend hospital phone call responsibility Reason for the Locum Need:

Provider out of office and possible FMLA Procedures Required (types of procedures to be performed by locum tenens provider): No procedures required of locum. Follow ups for established patients, monitoring patient status for planned chemo administration, monitoring patient response to established treatment and determining proper response. EMR System: EPIC
