Alexius Health Dickinson Medical Center needs 2-3 Anesthesiologist who are willing to complete all CRNA duties, participate in call rotation, and cover the following: Client prefers a provider who can also take call as needed. Providers with a wider range of availability will be prioritized.

DATES OF COVERAGE: Clinic: 7a-3:30p Sept 5-7, 12-13, 20, 27 Oct 2-3, 5-6, 11-12, 18-19, 24-26 Nov 2-3, 6, 8-9, 15, 20, 22, 30 Dec 7, 11, 13-14, 18, 20-21 On Call During the week: 7a-3:30p and Night call 3:30p-7a Sept 6, 8, 11, 13-15 Oct 2-4, 6 Dec 26-27 On Call Weekends: 7a-7a

• 24 hour call Sept 9 (7a) - 11 (7a) Sept 16 (7a) - 18 (7a) AUGUST-DECEMBER: OPEN AVAILABILITY (Please state your providers availability at name clear). Shift Types: Clinic: 7a-3:30p On call (during the week): 7a-3:30p and night call 3:30p-7a

• Night call must include 2.5 hours gratis On call weekends: 7a-7a, 24 hour call.

• Must include 4 hours gratis Rates to submit: Malpractice rates

• Must be included in the rates. Orientation rate (half the hourly rate, kicks in after 8 hours) - "Per hour of orientation or training beyond 8 hours" Hourly rate (Per hour, no OT) Night call rate (pager) 2.5 hours gratis - "Per night to hold the pager to include 2.5 hours gratis" Call back (Must Match hourly rate) - "Per hour of patient contact hours after gratis hours are completed, gratis prorate to MSA terms" 24-hour call rate (must include 4 hours gratis) - "Per 24 hours to hold the pager to include 4 hours graits.
  
  Holiday rates (1.5x the regular rate, per CS guidelines) Details: This job is in tandem with job 42903. The client has changed AP coding and doctors are no longer able to be presented to a mid-level job or vice versa.
  
  Patient demographic: Adult 99%; Pediatric 1% Average Number of Surgical Patients per Day- 7 Patient Types- General 35% Ob 22% Complex 32% Ophthalmology 11% (Complex ophthalmology describes a different supply item that the ophthalmologist uses for the procedure. • Independent model 6 OR's and 1 endoscopy suite. We run 3 rooms at a time.

• There are two main areas detailed below. Trio Submission requirements covers the information that must be entered into Trio to have the provider presentation presented to the client.
  
  Presentation submission requirements includes the information that must be sent over in the presentation packet to have your provider presentation reviewed and presented to the client. Strict adherence to these guidelines will allow for the fastest path to getting your providers accepted. To process the best presentations, Account Coordinators and Account Managers will kickback presentations that do not include the information below (if it has not been discussed already). If you are unable to include something in the presentation, detail why and the plan to get it sent over.
  
  Account Managers and Account Coordinators will take this into account as they review the presentation, though they still reserve the right to redirect it back to the vendor if they deem it necessary. Trio Submission requirements- the following must be updated in Trio Full legal name (first, middle, last) Suffix for provider (MD, DO, PA, NP, CRNA) NPI number must be entered in Trio Provider email and best phone number (this pulls to the cover page for Client) Best time to contact (this pulls to the cover page for the Client) Years of experience must be updated in Trio Years of locums experience is optional License
