DESCRIPTION

The Linkage to Care Specialist conducts targeted outreach, recruitment, linkage and care coordination to assist people in accessing screening and linkage to care for HIV/AIDS, Hep C, STIs, PrEP, mental health and/or substance use disorder. Promotes the program directly to the community, as well as other CBO's and medical providers. Additionally, can promote overdose prevention, including Narcan training and distribution, testing/screening for HIV, PrEP, STIs, Hep. C and submit monthly reporting.

Essential Duties, Responsibilities and Specialized Skills of a Linkage to Care Specialist I:

• Knowledge and experience working in the field of SUD, PrEP and/or HIV

• Conduct program intake including the social determinants of health, housing retention, risk assessment and service/action plans

• Provide health education, risk reduction and adherence education to patients

• Provide community testing and linkage to care

• Collaborate with the HIV, MAT, Medical and Behavioral Health Provider teams.

• Develop linkages and referral networks in the community for the purpose of referring patients for additional services and support

• Possess excellent communication skills in both verbal and written form

• Possess the ability to demonstrate cultural sensitivity in working with diverse populations

Minimum Education Requirement:

Bachelor's degree

Minimum Specialized Skills & Knowledge Requirement:

Bilingual English/ Spanish speaking strongly preferred

Salary: $22-27/hour
