A Linkage to Care Specialist acts as a friendly, helpful resource to clients and community partners. For clients who are newly diagnosed with HIV and/or are not currently in care, Linkage can answer any questions regarding living with HIV and disclosure of status, facilitate entrance into medical care, accompany clients to their first few appointments, make referrals to complementary services, and offer HIV testing to partners and friends among other things. As a part of their work with community organizations, Linkage builds and fosters relationships with agencies that offer support services, as well as those groups whose clients would benefit from AHF services.

A Linkage to Care Specialist plays an important role in the continuum of HIV care by bridging testing with medical care and other services. Working closely with testing programs, Linkage is available immediately after a reactive result to guide the client through next steps. In concert with healthcare centers, Linkage helps clients prepare for, and get oriented to their engagement in care, regardless of insurance status

By linking individuals to care and providing compassionate and non-judgmental customer service, Linkage to Care Specialists enable AHF to be of service to the community by providing support to individuals at a critical and potentially overwhelming moment. The ultimate goal of AHF's Linkage to Care Program is to assist clients through their HIV diagnosis and empower them to be proactive in taking care of themselves and protecting others.

As a paid staff member, you will support our loyal Red Cross disaster volunteers who serve selflessly to provide a bridge of assistance until individuals and families can resume a normal life. The American Red Cross of GA is seeking a Mass Care and Logistics Lead Specialist (Lead Specialist). This position will support the Metro Atlanta region and the surrounding counties. The Lead Specaislits work hours may vary and the selected candidate needs to be flexible to work on call 24/7 to cover disasters, attend weekend events and travel as requested for deployments, meetings and trainings. You must have a valid driver's license and good driving to operate a Red Cross vehicle. Our Mass Care and Logistics Lead Specialist is responsible for leading and managing a primarily volunteer team to support and provide Mass Care and Logistics in the Metro Atlanta and surrounding counties. The territory is based on the regional configuration. This position serves as the lead functional expert in a Red Cross Region for the Mass Care and Logistics functions. Tailoring national program offerings for the Region and communities within the Region, providing technical expertise, training, and leading regional implementation of Mass Care and Logistics, this position supports regional employees and volunteers in the disaster territories to build capacity in the local community for the disaster cycle. Specific functions of this position include program implementation, remote team building of employees and volunteers, and soliciting feedback from end customers. Responsibilities: 1. Implement strategies, initiatives, processes and procedures that support the successful region wide day-to-day and major relief operation provision of mass care and logistics services. 2. Within scope of position, represents the entire disaster cycle of preparedness, response and recovery. 3. Ensures American Red Cross services are available to diverse communities. 4. Works to ensure all Mass Care and Logistics activities are in compliance with region and national policies and procedures. 5. Communicates and implements Mass Care and Logistics priorities, goals and objectives. 6. Ensures American Red Cross services are available to diverse communities 7. Responsible for supporting consistent service delivery strategies throughout the region as well as supporting field teams implementation of strategies and plans. 8. Evaluates and reports on the capacity and effectiveness of Mass Care and Logistics services. Prepares recommendations for continuous improvement. 9. Ensures access to Safe and Well communication system for registration within the shelter environment. 11. Ensure functional needs shelter requirements are identified, resourced, and met. 12. Negotiates and manages regional mass care (feeding, sheltering, and bulk distribution) contracts and supports the negotiation of local contracts. 13. Serve as the subject matter expert (SME) and provide oversight for Logistics, in coordination with Supply Chain Real Estate Management (SCREM). 14. Serves as the subject matter expert (SME) and provide oversight for Mass Care role that Red Cross plays at both local level as well as in the co-lead role of ESF-6 within the National Response Framework. 15. Ensures roles and responsibilities of Red Cross identified in tribal, local, state and FEMA agreements (NMOUs, MOAs, etc) are being met. 16. Manages the National Shelter System reporting for both ARC and non-ARC shelters. 17. Manage disaster regional storage. 18. Coordinate the acquisition of Emergency Services materials, equipment and resources. 19. Coordinates specialized training in Mass Care and Logistics. 20. Establishes and maintains a peer-to-peer relationship with emergency management for coordination around Mass Care and Logistics. 21. Develops a primarily volunteer team to support the mass care and logistics activities. 22. Support field based staff and volunteer teams and systems. 23. Develops and cultivates professional relationships with key internal and external partners and organizations (assigned relationships monitored and tracked by a portfolio management system). Essential Functions/Physical Requirements: Depending on work assignment, may require ordinary ambulatory skills sufficient to visit other locations; or the ability to stand, walk and manipulate (lift, carry, move) light to medium weights up to 20 pounds. Requires good hand-eye coordination, arm, hand and finger dexterity, including ability to grasp, and visual acuity to use a keyboard, operate equipment and read technical information. Overall mobility is essential including sitting for long periods of time, driving a vehicle and working under challenging conditions. Work Conditions: Work is performed indoors with some potential for exposure to safety and health hazards related to emergency services relief work. May travel and participate in meetings and conferences throughout chapter jurisdiction, state and Red Cross system. Because the Red Cross is a disaster response organization, staff may be asked to be on 24-hour call during period of major disaster.
