Lifestyle Coach

Work Hours: Monday thru Friday, primarily daylight. One day per week will be a late start, roughly 10 AM - 6:30 PM.

Office Location: 3 days per week on-site at Peartree Way location (Beaver, PA). 2 days per travel to training sites. As program grows, additional training days will be added.

Under general supervision of the Diabetes Coordinator and Population Health Manager, provides lifestyle coaching and education to patients enrolled in the program. Individual will teach Center for Disease Control (CDC) and Medicare approved Diabetes Prevention Program (DPP) curriculum to eligible persons, coordinate with other instructors, ensure data integrity, establish relationships with physician practices and other referral networks to enhance awareness of the program. Support Population health by providing members high quality, cost effective, and easily accessible care and assist with any duties as deemed necessary to achieve this mission.

REQUIRED

High school Diploma. Highly motivated to help others with the ability to interact and communicate in a group setting.

Current certification in the group lifestyle balance program or obtain certification within 3 months. Able to work flexible hours. Excellent verbal/written communication skills and presentation skills.

Computer literacy and knowledge of Windows, Microsoft Office, Word, and PowerPoint. Successful completion of applicable clearances as outlined in Human Resources Policy HR-106 within 90 days of commencing employment.

PREFFERED

Associates degree in a healthcare related field; Experienced, certified Lifestyle Coach with familiar with the CDC and Medicare Diabetes Prevention Programs curriculum. Experience in a healthcare setting.
