LifeLong Medical Care AmeriCorps Health Fellows

Position will consist of: Health Education/Health Coaching/Care Coordination Improve utilization of Health Services and Programs, such as providing education in one-on-one or group settings (AmeriCorps members do not provide health services related to prohibited items), assisting individuals with health-related services/programs, and following up as needed.

Case Management/Patient Navigation, such as: assisting individuals with the enrollment process for health programs and health insurance plans. Community Engagement & Outreach by conducting outreach efforts to educate the community about resources, programs, and healthcare options. Service Projects: coordinate, plan, and participate in various community service projects.

Volunteer Recruitment and Training/Management: assist with recruitment, training, and retention of non-AmeriCorps volunteers; assist with the management of non-various health topics and professional development. Further help on this page can be found by clicking here.

Member Duties : As an AmeriCorps fellow, you will serve side-by-side with health center staff and provide culturally and linguistically appropriate health education, outreach, referrals, and enrollment into programs directly to community members while getting the skills you need to pursue a health-related profession.
