The LVN has the responsibility of total resident care for independent and assisted living residents. The LVN performs professional nursing duties in accordance with the Masonic Homes and state RCFE regulations. This position reports to the Director of Health Services and may be required to supervise other staff positions.

JOB RESPONSIBILITIES

A. Essential Functions:

  • Responsible for nursing care, medication administration and treatments for the assisted living residents in compliance with Federal & State regulations and the Masonic Homes guidelines.
  • Responsible for the admissions of new residents to include all admission assessments, the assessment of resident health status for both new and existing residents, the subsequent development of the service plan and documentation.
  • Recognize changes in conditions of residents and notifies physicians and/or family members of any change in resident's health and provides proper documentation.
  • Performs periodic audits and reviews clinical systems to ensure compliance with State regulations and Masonic Homes policies and procedures as assigned.
  • Initiates writing and updating Resident Assessment and Service Plans as residents condition changes, in collaboration with active living, social services, and dietary services.
  • Supervise resident care personnel on shift in performing duties by checking their work to be sure that assignments have been completed
  • Maintains close communication with all departments and staff members, Health Services Director, health care professionals and families about the needs of residents.
  • Notifies MD of resident changes and concerns after consultation with RN;
  • Personally receive or place calls to physician, receives and processes MD orders, and order medications as needed.
  • Monitors medications of residents as ordered by MD;
  • Administer intramuscular, intradermal, and sub-cutaneous injections.
  • Chart medications and treatments according to procedure
  • Verify inventory of drugs covered by Controlled Substances Act of 1970
  • Ensure that individual Resident Assessment and Service Plan is followed
  • Attends family conferences;
  • Understands roll in the Safety & Disaster Plan.
  • Performs other duties as assigned.

General Responsibilities:

3. Follows oral and written instructions accurately.

4. Keeps supervisor informed of work priorities and problems.

5. Responds appropriately to emergency situations and disasters.

6. Provides a safe environment for patients, visitors and staff.

7. Assures security of environment to prevent exit of wandering residents. Implement the facility procedure for elopement if any resident leaves the facility without permission.

8. Follows facility standards relating to customer service.

9. Maintains confidentiality of residents and other work-related issues.

10. Assures that all residents are treated with respect and dignity.

11. Follows facility rules of conduct.

12. Attends in-service related to work and completes required in-service annually; attends outside classes to enhance role and professional standards, as appropriate.

13. Provides timely notification of illness/absences to appropriate supervisor following departmental policy and time line.

14. Attends facility meetings as assigned.

15. Follows dress code for work area.

16. Performs other duties as assigned.

Requirements

Graduate from an accredited nursing program.

One or more years of long term care nursing experience as a licensed nurse.

Currently licensed as LVN to practice in the State of California

Basic Life Support (BLS) certification

This position reports to the Director of Health Services. </p><p><strong>ESSENTIAL FUNCTIONS:</strong></p><p><strong>A. C.N.A.</strong></p><p>1. The Assistant provides personal care assistance to residents as directed: (bathing, grooming, feeding, and toileting)</p><ul> <li>Demonstrates ability of administering complete/partial resident baths. This would include bath, oral care, dentures, Foley care, hair/nail care, and shaving, bed making and cleaning of utensils used.</li> <li>Assists in the toileting needs of the residents, records outputs accurately. Empties drainage collection devices and completes documentation of BM Book, toileting scheduling, Rehab and nourishments.</li> <li>HIPAA Competency – Protects resident’s privacy by insuring resident charts or items with resident data with protected health information (PHI) is not left unattended or left opened.</li> <li>Demonstrates ability to read, understand and follow individual resident care plans.</li> <li>Maintains harmonious relationship with co-workers, providing support and assistance to co-workers to ensure quality resident care.</li> </ul><p> 2. Prepares resident for meals; serves and collects trays, assists in setting up and feeding residents; serves fresh water and nourishments when indicated.</p><ul> <li>Completes meal assessment sheet and records oral intakes accurately as needed </li> <li>Utilizes proper lifting and body mechanics to prevent personal injury</li> <li>Demonstrates attitude of cooperation and professionalism when working in the Department</li> <li>Organizes work to complete assignments within the required time frame.</li> </ul><p>3. The Certified Nursing Assistant helps residents with transferring and lifting, raising residents in beds and turning residents. </p><ul> <li>Maintains confidentiality of all resident, Facility and/or physician related information</li> </ul><p> Maintains resident confidentiality and does not discuss resident in public areas, such as cafeteria, hallways, elevators, etc.</p><ul> <li>Demonstrates ability to lift, transfer and transport residents in wheelchair, bed or car, etc. Utilizes appropriate transfer devise.</li> <li>Reports problem areas to the Supervisor</li> <li>Identifies when help is required in providing resident care and seeks it appropriately recognizing own limitations.</li> </ul><p>4. Obtains temperature on residents whose medical conditions warrants such monitoring. Obtains urine samples, weights, </p><ul> <li>Demonstrates ability to obtain and record resident’s temperature, pulse, respiration, weight, as directed by nurse. Demonstrate the ability to assist with specimen collection (urinalysis, etc.)</li> <li>Conducts job duties in accordance with departmental standards</li> <li>Recognizes and reports changes in resident conditions to a nurse, demonstrates knowledge of complications and asks appropriate questions to clarify the situation </li> <li>Verbal ability to express ideas and views effectively. Must possess the ability to making independent decisions when circumstances warrant such action.</li> </ul><ul> <li>Reports accurate results to achieve the best possible resident care</li> </ul><p>5. Regularly checks on residents insuring comfort, dignity and privacy </p><ul> <li>Answers call lights promptly; responds promptly to resident inquiries, reports any abnormal findings or complaints to primary nurse.</li> <li>Demonstrates mission of the Facility by providing quality care in a personal manner with total regard for individual human dignity</li> <li>Communicates clearly and tactfully with residents and their families</li> <li>The Certified Nursing Assistant cooperates with other personnel to achieve department objectives and maintains good employee relations, interdepartmental objectives, and infection control policies.</li> </ul><p>6. Attends all in-services and meetings as required</p><ul> <li>Projects poise, authority, and a positive image. Wears appropriate clothing and accessories, giving proper attention to personal hygiene. Wears Facility ID badge while on duty.</li> <li>Infection Control Competency: Demonstrates knowledge of and adheres to all infection control procedures including proper hand washing techniques, contact with blood spills and standard precautions</li> <li>Attends all mandatory in-services. Attends department meetings as required and scheduled </li> <li>Competency: Participates in drills and demonstrates knowledge of Plan for Department</li> </ul><p>7. Cleans each unit as assigned by the Director, including, ice bucket/ladle, wheelchair/carts</p><ul> <li>Conducts job duties in accordance with departmental standards</li> <li>Assists in the containment of the Facility’s costs through conserving so supplies and equipment</li> <li>Organizes work to complete job tasks in a timely manner. Responds promptly to change in workload, adjusting assignments to achieve optimal productivity and efficiency</li> <li>Straightens and cleans utility room/pantry as assigned per unit/shift. Delivers supplies to resident rooms. Completes weekly cleaning assignments as directed. Maintains resident rooms in a neat orderly and clutter free environment. Ensures that closets, drawers, clothing and personal items are labeled</li> </ul><p>8. Perform other related duties as assigned or requested</p><p><strong>B. MEDICATION TECHNICIAN</strong></p><ol> <li>Provide routine daily medications, either prescription or non-prescription, to residents whose condition and drug regimen are stable. </li> <li>Must ensure the resident actually swallows the medications. </li> <li>Report any observed a change in the resident's vital signs or behavior, or any other indication of adverse effects from a medication, to the charge nurse. </li> <li>Assist residents with time reminders, opening containers and making sure proper dose is taken by resident.</li> <li>When a resident is on an antibiotic, the resident’s temperature will be checked once per shift.</li> <li>Report any adverse reactions from medications.</li> <li>Will ensure that the resident has a 72 hour supply of medicine on hand at all times. </li> </ol><p><strong>C. ESCORT</strong></p><ol> <li>Escorts or transports residents to their medical appointments or other locations.</li> <li>Determines resident’s name, destination, and mode of travel, time, and other data, following written or oral instructions. </li> <li>Prepare residents for transport and assist them in walking to prevent accidents by falling, or transports non-ambulatory resident, using wheelchair.</li> <li>Carries resident's personal effects as needed. </li> <li>Responsible for notifying LVN on duty upon arrival or departure of residents. </li> <li>Report immediately any unusual symptoms of residents i.e., fainting or shortness of breath during time of escort.</li> <li>Assist in the loading and unloading of residents onto vans as needed.</li> <li>Transports resident, alone or with assistance, in a wheelchair, walker, etc. to designated areas within facility during appointment. </li> <li>Provide and adjust equipment for safety and comfort of the patient i.e., O2 tank, etc. </li> <li>Attends to the care of documents relating to the resident e.g., medical charts</li> <li>Follow safety precautions and infection control techniques.</li> <li>Perform first aid and CPR during emergency situations.</li> <li>Keeps Manager/Supervisor informed of activities, needs and/or concerns;</li> <li>Perform other related duties as assigned or requested</li> </ol><p><strong>Requirements</strong></p><ul> <li>Completion of 10th grade (High School)</li> <li> 2 or more years of experience as a C.N.A. in long term care</li> <li> Successful completion of a Certified Nurse Aide program ; · Medication Aide Certification · Active and current C.N.A.
