The LVN is part of the team concept in a clinical setting for the delivery of healthcare. This position oversees client medications via monitoring, assessing clients for mental health, builds rapport with clients, follow-up on medications and doctor appointments.

Requirements

All direct care staff shall have graduated from high school or possess a GED and have a minimum of one (1) year of full-time experience, or its part-time equivalent, working in a program serving persons with mental disabilities. Such experience must be in the direct provision of services to clients. If the employee does not have the required experience, the program shall document a specific plan of supervision and in-service training for the employee which will guarantee the ongoing qualification of the employee to perform the job. The plan should include but not be limited to the frequency and number of hours of training, the subjects to be covered, and a description of the supervision to be provided.

● High School Diploma required, Bachelor's Degree preferred.

● Current LVN license in State of operations required.

● CPR and First Aid Certified.

● Two years of experience as an LVN or in a health-related field.

● Knowledge of basic principles, practices and techniques in mental health.

● Knowledge of medications effects, indications, and contraindications.

● Ability to read and interpret written information; write clearly and informatively; edits work for spelling and grammar. Ability to speak clearly and persuasively in positive or negative situations; listens and gets clarification; responds well to questions; demonstrates group presentation skills; and participates in meetings.

● Working knowledge of Microsoft Word, Excel, and Outlook.

● Must be 18 years of age

● Must have required health screening

● Must have current TB

● Must have criminal record clearance

● Follow Physician's instructions on medication management protocol and procedures.

● Monitor clients physical and emotional well being; vital signs checked per physician order.

● Monitor client’s vital signs for signs/symptoms of medication reaction.

● Write notes and reports, as protocol and procedures indicate.

● Document and report on client’s status to team members.

● Complete nursing assessment on clients admitted to a social residential transitional program.

● Complete transcription of doctor’s orders pertaining to modified protocols.

● Assist clients with all self-administered medications via monitoring as directed by the appropriate health care professional

● Assist clients with personal hygiene.

● Attend job-related meetings, in-service training, staff development training, participates in team problem solving methods, building rapport, respond to requests, and maintains a cooperative relationship among the health care team by communicating information.

● Maintains nursing supplies inventory by checking stock to determine inventory level, anticipating needed supplies and inform the Administrator so that they may place order(s) for supplies.
