POSITION SUMMARY

The Licensed Practical/Vocational Nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.

DUTIES AND RESPONSIBILITIES

  • Provides direct patient care as defined in the State Nurse Practice Act.

  • Implements plan of care initiated by the registered nurse.

  • Provide accurate and timely documentation consistent with the plan of care.

  • Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.

  • Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, SW, ST, Dietician) to the registered nurse and/or Clinical Supervisor/Nursing Supervisor.

  • Uses and prepares equipment and supplies effectively, adhering to aseptic technique.

  • Provides appropriate pain/symptom management. Evaluates patient's response to treatments/medications.

  • Participates in personal, professional growth and development. Also participates in organization's quality assessment performance improvement teams and activities.

  • Performs other duties as assigned by the registered nurse.

POSITION QUALIFICATIONS

Graduate of an accredited practical nurse or vocational nursing program. Three years of nursing experience. Community health/hospice or medical/surgical experience is preferred. Currently licensed as a LPN/LVN in the State. Complies with accepted professional standards and practice. Demonstrates good verbal and written communication, and organization skills. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Possesses and maintains current CPR certification. Understands philosophy of hospice concept. Also understands needs of the terminally ill.

Qualifications Education for Registered Nurse: * Graduation from an accredited school of nursing Experience for Registered Nurse: * Minimum two years of correctional/detention facility experience Licenses/Certifications for Registered Nurse: * Current (and must maintain) unrestricted Registered Nursing license, certification or registration in state of practice * Current (and must maintain) lifesaving BLS certification through AHA Responsibilities Utilizes the nursing process to provide quality and efficient patient care according to established standards including: accurately completing admission and other assessments, and collecting, analyzing, and prioritizing assessment data to facilitate proper patient careo Transcribe physician, PA/NP orders o Document nursing assessments using the S.O.A.P.E. format o Prepare and administer medication in accordance with physician orders including: immunizations, antibiotics, narcotic pain medications, vitamins, topical agents, insulin, blood thinners, and allergy antigens. o Initiate standardized procedures, orders, i.e., detox, opiate, alcohol o Perform 14-day physicals and sick calls as required Perform laboratory procedures and testing to include: blood glucose with visual or meter interpretation; interpretation of skin testing results; dipstick urine; urine specific gravity Obtain specimens for laboratory testing including but not limited to: venipuncture; straight catheterization for urine collection; throat culture; sputum specimens; stool specimens Coordinate the activities of his/her shift, provides timely and adequate supervision to the LVNs and Medical Assistants, responds to the intake area as requested and coordinates/communicates with detention staff as needed Maintain a clean, safe, and secure working environment Triage and communicate with physician, PA/NP, or Nursing supervisor any potentially significant medical complaints or observations discovered during routine duties Respond and initiate lifesaving measures to man-down calls, identify medical emergencies, and arrange for transportation to hospital May assist in the job shadow portion of the interview process, orientation and training of new and potential staff May participate in performance evaluations and training of other team members Perform other duties as assigned.
