Position Description:

The Licensed Vocational Nurse (LVN) will be responsible for providing care and services for a group of hospice patients, visiting patients where they reside, which could include private homes, assisted living facilities and hospitals. This position will be after hours on-call working seven days/nights on and seven nights off.


  • Current Licensed Vocational Nurse (LVN) licensure (in good standing) in the state(s) of practice with one year of experience under license

  • Admissions and On-call experience also preferred

  • Works well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.

  • Work closely with patients on a 1-1 level but get strong support from a patient care team.

  • Can work independently and on your own from a day-to-day basis with activities which include: phone, email and written communication, meetings with a variety of healthcare professionals, and an understanding of hospice rules and regulations.

  • Time management skills to manage visits, documentation, meetings (i.e.: to scheduled visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns)

  • Ability to learn an electronic medical records system.

  • Reliable transportation, valid drivers license, and auto insurance, to be able to travel across the service area

Position: Licensed Vocational Nurse Status: Part Time ( 20 hours/week) Schedule: Sunday & Thursday | Shift: AM (7:30am-11:30am) 4hrs/day Location: Fremont, Ca Duties: * The Licensed Vocational Nurse performs general nursing procedures, behavior management, direct care, and training as dictated by the individuals' health and behavioral care needs. * Performs nursing procedures as authorized within the scope of practice. * Participates as a member of the interdisciplinary team and contributes to the development of the Individual Support Plan. * The LVN may function as lead staff on the shift and may oversee DSPs and/or CNAs. * Responsible for providing active programming at all times. * Responsible for adherence to standards, procedures, and schedules established by their supervisor. * Responsible for ensuring that any suspicion of or knowledge of suspected abuse is reported in accordance with the law and program policies "Mandated Reporter" (SOC 341). Requirements * Possess and maintain a valid, active license from the State of California to practice as a Licensed Vocational Nurse. * Minimum 1-year experience with Human Services field preferred. * High School diploma * Valid California driver license * Successful completion of pre-employment physical and drug screen, including current TB * Successful completion of DOJ and FBI background check * Successful completion of all training as required by applicable regulations, including, but not limited to, DSP1 and DSP2. Working Environment/Hazards * Ability to work in a variety of environments and settings. * Ability to work weekend shifts Hours of operation is 24-7; AM-shift, PM-shift, and NOC-shift. * Potential exposure to communicable diseases, blood-borne pathogens, medicinal preparations, and other conditions common to a clinical nursing environment. * Interaction with consumers who display aggressive and/or self-injurious behaviors. .
