Status: Full-Time

Wage: 60k - 75k DOE + Benefits

Primary Objective of Position:

This position plays an important role in the ongoing care coordination of residents across Jamboree communities throughout California and clients participating in homeless services programs. This position will work collaboratively with Program Managers, Supportive Services Coordinators and Lead Care Managers. As part of a team, this role will assume overall responsibility for medication management, assessments, consultation to staff and residents, recommendations for the development of a comprehensive Care Plan and teach appropriate interventions and training to staff. The Licensed Vocational Nurse (LVN) will provide care to residents and clients from diverse backgrounds, from those transitioning from homelessness, families, and Seniors.

Major Areas of Accountability:

  • Collaboration with the Community Impact team, resident, physician, and/or other health care providers to develop solutions for individual to get their needs met and stay housed.

  • Meet with residents struggling to maintain housing, possibly due to mental health or medical related issues, to assess the need for higher level of care of additional supports needed to maintain housing.

  • Review of all medications prescribed by all prescribers providing care to the patient, and any over the counter and herbal products the patient may be taking to identify and address medication concerns. Problems may include medications not being used correctly, duplication of medications, unnecessary medications, and the need for medication(s) for an untreated or inappropriately managed condition.

  • Assist residents in identifying elements/benefits to their health care plans.

  • In collaboration with assigned Community Impact team staff, support the development of Care Plan to achieve optimal levels of health and self-management.

  • Facilitate linkage to appropriate community resources to address social determinants of health.

  • Understand medications and their actions and educate the resident on which medications need to be taken.

  • Provide age-specific, culturally competent, ethical care within legal standards of practice.

  • Use problem-solving and conflict resolution skills to foster effective work relationships with team members.

  • Actively participate in team coordination meetings, trainings, and staff consultation requests.

  • Maintain required certifications.

  • Other duties as assigned to support the mission of the organization.

Level of Education, Experience and Skill Set:

  • LVN certification and license in CA.

  • 3-4 years' experience working in a medical setting.

  • Must possess excellent interpersonal skills particularly with family cultural/sensitivity.

  • Current CPR certification.

  • Ability to build relationships and teams.

  • Ability to take the initiative and independently respond to requests in writing, in person, or over telephone.

  • Ability to accurately manage timely data entry and file management requirements.

  • Familiarity with database systems and electronic health records (EHR) systems.

  • Strong knowledge of the complexity of poverty homelessness, mental illness, trauma, substance abuse, health issues, and older adults required.

  • Strong interpersonal, written, and oral communication skills; proven ability to communicate with diverse audiences.

  • Experience with individuals with serious mental illness and substance addiction preferred.

  • Valid California driver's license and availability of an insured vehicle.

  • Must be able to travel between sites 50%.

  • Job includes some evening and weekend hours and travel throughout California.

Physical Demands:

The LVN is required to talk and hear. The employee is often required to sit and use their hands and fingers to handle or feel. The employee is required to stand, walk, reach with arms and hands, climb, balance, twist, lean, move from one location to another and to stoop, kneel, crouch or crawl. Vision abilities required by this job include close and far vision. Ability to operate a computer keyboard, and lift files and reports. Exposure to glare from a computer. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Must be able to lift 25 lbs. and travel via automobile 75%.

Work Environment:

The work environment will usually be in a professional office setting, with possible shared workspace. The noise level is usually quiet to moderate. When traveling the environment will change dependent upon the site visiting.
