Job Title: Licensed Vocational Nurse (LVN)

Pgm/Dept: Transitional Residential Treatment Program

Reports to: Program Director

Classification: Regular, Full-Time, FLSA Non-Exempt

Pay Range: $28.85 - $37.00 hourly

Location: Berkeley, CA 94709

Schedule: Day and Swing Shifts Available

Position Summary

The LVN is responsible for completing assigned nursing-related services to each client and for linking clients to physicians and outside medical resources. They inform the clinical team about medical issues and are available for emergency medical situations. The nurse provides all services under the Program Director's or Medical Director's supervision. The nurse is responsible for assisting with medical and history screens, providing routine nursing care, and assisting the Program Director in overseeing the medical and psychiatric management of the client in coordination with the attending physician.

Primary Responsibilities

  • Patient Care:

  • Administer medications and treatments as prescribed by healthcare providers.

  • Monitor clients' vital signs and report any significant changes to the healthcare team.

  • Assist with activities of daily living (ADLs) and provide supportive care to clients.

  • Conduct health assessments and contribute to the development of individualized care plans.

  • Collaborate with other healthcare professionals to ensure holistic care delivery.

  • Health Education:

  • Educate clients on medication management, health maintenance, and self-care techniques.

  • Provide guidance on nutrition, exercise, and healthy lifestyle choices.

  • Offer support and education to clients and their families regarding their mental health and substance use disorders.

  • Documentation and Reporting:

  • Maintain accurate and up-to-date medical records, including nursing assessments, treatment plans, and progress notes.

  • Document all nursing interventions, medication administration, and client responses.

  • Communicate effectively with the healthcare team regarding clients' progress and any concerns.

  • Crisis Intervention:

  • Respond promptly to medical emergencies and provide immediate care as needed.

  • Implement crisis intervention techniques to de-escalate situations and ensure the safety of clients and staff.

  • Collaborate with the treatment team to develop and implement crisis management plans.

  • Quality Improvement:

  • Participate in quality improvement initiatives to enhance the delivery of care and achieve program goals.

  • Identify opportunities for process improvement and contribute ideas for enhancing patient outcomes and satisfaction.

  • Perform other duties as assigned.

Qualifications and Requirements

  • Licensed as an LVN in the state of California and maintains current licensure per state requirements.

  • Maintains current CPR certification.

  • Prior experience working in a behavioral healthcare setting preferred.

  • Must be able to meet and receive a criminal records clearance, as required by California Code, Welfare and Institutions Code - WIC § 5405, federal contractor requirements and/or agency practices.

  • Must pass post-offer, pre-employment physical and TB clearance as required by state community care liecensing standards.

  • A valid driver's license, reliable personal vehicle, personal auto insurance as required by law, and an MVR sufficient to obtain and reasonably maintain insurability under agency auto liability policies are essential job requirements.

  • Local travel as required to fulfill primary job responsibilities and duties.

Knowledge Skills & Abilities

  • Strong knowledge of psychiatric nursing principles and practices.

  • Knowledge of recovery-oriented principles, harm reduction, trauma-informed care, and evidence-based therapeutic modalities.

  • Ability to effectively intervene in crisis situations using de-escalation techniques.

  • Excellent verbal and written skills and the ability to present information effectively.

  • Excellent interpersonal, negotiation, and conflict resolution skills.

  • Proficiency in computer applications for documentation and record-keeping.

  • Excellent organizational skills and attention to detail.

  • Strong analytical and problem-solving skills.

  • Ability to prioritize tasks and to delegate them when appropriate.

  • Ability to act with integrity, professionalism, and confidentiality.

  • Ability to build supportive and respectful working relationships with staff, participants, and stakeholders in the community.

Physical Requirements

Physical demands include continual sitting, typing, and repetitive computer use for up to 8 hours a day. The position also requires frequent communication with others, including hearing and speaking clearly with individuals and groups, in person and via phone and video calls. Ability to lift 15 pounds at times. Regular local travel and operation of a motor vehicle is required. These requirements are representative of those that must be met to successfully perform the job's essential functions.
