Corpus Christi Medical Center is currently looking for a Licensed Social Worker at our campus.

Position Summary:
Develops, plans and implements services to patients, families, the community, and staff for the social emotional, psychological, economic, and environmental issues related to the entire continuum of care. Responsible for development and maintaining on-call processes for social services support in the hospital.

Responsibilities:
1.Formulates a psychosocial assessments in order to share with other multi-disciplinary members to enhance the effectiveness and outcome of plan-of-care
2.Be a resource to patients/families confronting simultaneously illness and/or divorce, custody, and adoption issues
3.Readily provides crisis intervention, counseling , and emotional support where and when needed within the hospital; provide counseling and psycho education around care giving role, end-of-life, grief and bereavement issues; facilitates completion of DNRs, Advanced Directives, and formal Health Care proxy Appointments.
4.Develops professional relationships with, and knowledge of aftercare resources, including their scope and referral process requirements to improve the safety of the discharge planning process and reentry of the patient into the community (i.e. State and regional forms of assistance, housing, and entitlements). Maintain an updated resource list.
5.Participates in treatment and discharge planning develops a thorough and current understanding of criteria established by payer sources to obtain reimbursement.
6.Available to all floor staff when unscheduled consultation is required in person of by telephone, as dictated by level of priority.
7.Identify root-causes of frequent re-admissions (>31 days) and address them whey they fall within the scope of Social Work (i.e. culturally based perceptions of illness and health, ill-understood nature of life-changes secondary to disease, lack of resources). Establish triggers for pre discharge family meeting, including physician, nurse, or educator
10. Develop and offer support groups to the larger community to enhance self-care behaviors, education on prevention and health maintenance, management of care giving role.

Qualifications:

Required Minimum Education

BSW or MSW

Required Education qualifications include

BSW or MSW based on license requirements of the state in which the Hospital operates. MSW or LCSW preferred. Certified/Accredited Case Manager (ACM) preferred

Required License/Certification

LMSW or LBSW

What you will do in this role: Applies evidence-based methods and theory to the assessment, diagnosis, and treatment of mental, emotional, and behavioral disorders for individuals and families. Responsible for assessing patients to determine goals and priorities for counseling; provides age-specific treatment for patients. Collaborates with physicians, nurses, and other clinical disciplines in making decisions and coordinating each patient's care. * Provides psychosocial assessments and treatment to individuals with mental disorders, as well as various behavioral and emotional disturbances. * Provides individual, group and family counseling. * Provides crisis intervention counseling as necessary. * Serves as a member of a multidisciplinary medical treatment team. * Maintains liaison with area resources and coordinates social services for discharge planning. * Provides consultation on behavioral health aspects of procedures, policies, and services for volunteers, medical staff, community groups, and patients. * Recognizes the role of patient in treatment planning. * Participates in general department meetings, treatment team, and clinical supervision meetings. * Plans, manages, and provides case management from admission through discharge. What qualification you will need: * Master's degree in social work or counseling from an accredited institution. * Licensed in the state of Texas as a Licensed Clinical Social Worker, Licensed Professional Counselor, Licensed Master Social Worker, or Licensed Professional Counselor Intern. * Computer skills required; must be able to document in electronic medical records. * Exercises knowledgeable, independent clinical judgment on routine and non-routine matters. * Must be able to work within a multidisciplinary treatment team in a fast-paced urgent care environment.
