We are currently seeking a Licensed Social Worker for our Treatment Foster Care (TFC) Program. Woodbourne Treatment Foster Care (TFC) social workers provide youth with safe homes, clinical case management, clinical individual therapy, and access to mental health services while they are living in a supportive family setting. Our social workers provide youth with trauma-informed, individualized, and strengths-based treatment. Family therapy, parenting classes, and in-home behavioral supports are among the available services that can be provided within the Woodbourne TFC program.

COVID-19 Precautions: The safety of our youth, families, employees, and candidates are a top concern. We are following the guidelines provided by the Centers for Disease Control and Prevention and adapting our processes as changes arise. In an effort to limit physical contact as much as possible all interviews will be conducted via phone or virtual meetings (Zoom).
Facilitate quarterly treatment planning meetings and develop an individual treatment plan that provides on-going assessment and diagnosis.
Supervise the placement and ongoing support of youth placed in foster and adoptive homes assigned to their caseload.
Work effectively with and engage biological and adoptive families in the youth's treatment process.
Develop treatment plans and utilize a variety of treatment approaches with clients.
Communicate with families, legal systems, schools, mental health providers, probation and social service agencies and provide court testimony as required.
Maintain appropriate documentation and files to support the services that are provided to families.
Participate in staff and supervision meetings, and required Woodbourne Center in-service meetings.
Understands and supports the Woodbourne Center and Nexus Philosophy and mission.
Ensure confidentiality of all treatment and services information pertaining to clients and their foster, adoptive and biological families.
Perform other job related responsibilities, tasks and projects as needed.
