Queens-Long Island Renal Institute, located on the lobby level of the renowned Parker Jewish Institute for Healthcare and Rehabilitation in New Hyde Park, New York, is seeking a Full Time Social Worker, with a strong background in Renal Social Work, to join our team of and provide quality clinical care and support to our patients and their families.

This position will have a regular schedule of 10am-8pm, 4 days per week.

QLIRI provides state-of-the-art in-center hemodialysis treatment for kidney disease patients. Our experienced interdisciplinary team of nephrologists and renal nurses offer chronic dialysis services in a comfortable and modern setting. Located on the Queens-Nassau boarder, Queens-Long Island Renal Institute has a longstanding reputation for the finest quality of care, state-of-the-art technology and uncompromised dignity.

Key Tasks Include:

  • Conducts initial patient psychosocial assessment and comprehensive plan of care

  • Provides support, information and assistance in treatment planning

  • Develops and continues to monitor psychosocial goals for plan of care

  • Counsels new patients and families on pre-dialysis education, including treatment options

  • Acts as advocate on behalf of patient and family

  • Completes and documents initial and ongoing comprehensive assessment, evaluations and quarterly notes, along with documenting and monitoring ongoing psychosocial issues and patient goals

  • Facilitates and/or refers patients and families to support/education groups

  • Facilitates education on Advance Directives and treatment modalities

  • Coordinates and participates in patient/family care conferences with interdisciplinary team

  • Analyzes and provides information and facilitates referrals to community, private, state and federal agencies based on psychosocial needs

  • Provides crisis intervention expertise as necessary

  • Provides assistance to patient and family in adjusting to dialysis and altered lifestyle, and provide resource information

  • Arranges patient travel arrangements for both guest and permanent patients and ensure completion of arrangements

  • Coordinates patient transfers/ visit to other facilities

  • Provides assistance to patient regarding health insurance

  • Maintains patient confidentiality and administers patient care in a respectful manner

Position Requirements:

  • Licensed Social Worker in New York State; LMSW required

  • At least two (2) years of prior work experience as a Social Worker; prior dialysis social work experience a plus but not required

  • Strong interviewing and analytical skills for patient evaluation, problem solving, and recording

  • Capable of communicating clearly and effectively with all types of patients and their families

  • Professional, flexible and a team player

  • Fully Vaccinated against COVID-19

Hemodialysis RNs at Queens Long Island Renal Institute are responsible for performing an age specific plan of care for a designated group of patients using the nursing process of assessment, diagnosis, outcome identification, planning, implementation and evaluation of patient care. They collaborate with physicians and other health team members in coordinating and implementing procedures and treatments and use leadership skills/clinical judgment in coordinating patient care and directing/delegating activities of the patient care unit team. KEY TASKS: * Assesses the patient's physiologic health status * Assesses patient's psychosocial health status * Identifies patient outcome * Formulates the plan of care and establishes the priorities necessary to achieve expected outcome * Implements the plan the care * Evaluates care provided for patient outcome * Collaborates with other care team members in planning and carrying out treatment regimen Provides direction to other members of the care team * Administers medications correctly and safely * Participates in patient and family education The ideal candidate will have an RN license in NYS and will be a graduate from an accredited nursing program (BSN preferred.)
