What You Will Do

As an Associate with Saint Thomas Health Svcs-TN, you will have the opportunity to coordinate the overall interdisciplinary plan of care for patient, from admission to discharge.

Responsibilities:

  • Creates plan for care across the continuum, integrating patient/family preferences and values.

  • Monitors patient care through assessments, evaluations and/or patient records.

  • Advocates for resources and removal of barriers.

  • Maintains ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.

  • Acts as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care.

What You Will Need

Licenses/Certifications/Registration:

  • Required Credential(s):

  • One or more of the following:

  • Clinical Counselor obtained prior to hire date or job transfer date.

  • Bachelor Social Worker credentialed from the Tennessee Board of Social Workers obtained prior to hire date or job transfer date. (LBSW OR LMSW)

  • Preferred Credential(s):

  • Case Manager credentialed from the American Case Management Association (ACMA).

  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC).

Education:

  • Diploma

  • High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degree required

Work Experience:

  • None required.

Develops policies and procedures for program operation and prepares reports and statistics for facility, VISN, and national use. The incumbent may be responsible for the program's budget and various control points assigned to the program within the assigned areas. Duties include but not limited to: * Develops and implements the Caregiver Support Program specific to his/her area of assignment and insures compliance and collaboration with local facility and the VA Central Office Caregiver Support Program initiatives. * Administers information and analytical systems to evaluate and enhance the quality of service provided to caregivers, families and Veterans. * Develops and implements information systems to track service activities including visits, referrals, caregiver demographics, equipment utilization and caregiver and Veteran satisfaction. * Participates in direct caregiver and Veteran contacts to identify and assess caregiver stress/burden and the development and application of directed clinical treatment interventions. * Advises and collaborates with interdisciplinary teams throughout the medical center on caregiver issues. * Provides ongoing consultation and support to colleagues regarding caregiver support. * Develops and facilitates caregiver support groups and educational programs.
