Position Purpose: In compliance with the Arizona State Nurse Practice Act, and under the direction of the RN, assist with implementation of hospice plan of care for each assigned patient.

Hours/Shift: Full Time/Days with a 4/10 schedule

Key Responsibilities:

  • Under the direction of the primary RN, complete in-home care to assigned patients as designated in the Home Health Plan of Care (POC).

  • Record patient responses to treatment/procedures and note any changes or variances.

  • Complete accurate and thorough documentation of patient care visit within twenty-four (24) hours of visit according to departmental, state, and federal regulatory requirements.

  • Initiate communication with the primary RN, attending physician, and other disciplines of the Home Health Team (IDT) to report patient responses to interventions, significant changes, and to coordinate care and use of resources for the patient/family.

  • Attend, participate in, and contribute to team conference meetings in an effort to achieve quality patient outcomes.

  • Participate in departmental and organizational initiatives for process improvement and quality assurance, including ongoing professional development.

  • Comply with all federal, state, organizational, and departmental policies, procedures, and requirements.

Qualifications:

  • High School diploma or equivalet

  • Completion of LPN program

  • Current AZ LPN License

  • BLS through American Heart Association or American Red Cross

  • Level One Fingerprint Clearance Card, or application, required within 20 days of hire

  • Valid AZ Driver's License

Work Requirements:

  • Requires travel throughout local service area and interactions with customers in their home setting

  • Duties have routine potential for exposure to blood, body fluids or tissues; ability to move patients >300lbs; sit, stand, walk, and bend duration of work shift

  • Ability to care for patients with infectious diseases and in a variety of settings
