Overview

Our Licensed Practical Nurse assists with the provision of comprehensive psychiatric and medical care within the scope of their practice and under the supervision of a Registered Nurse. The Licensed Practical Nurse makes and receives assignments, sets priorities and participates in program services as assigned.

Qualifications

Education for Licensed Practical Nurse:

  • Graduate from an accredited nursing school

Experience for Licensed Practical Nurse:

  • One (1) year of experience in an acute care and/or forensic setting preferred

Licenses/Certifications for Licensed Practical Nurse:

  • Must hold a current, unrestricted nursing license in the state of practice or qualify for licensing through reciprocity arrangements with other states

Responsibilities

  • Assists in providing comprehensive nursing care services including; treatment, recovery plan development, implementation of the plan of care and evaluation of the effectiveness of nursing intervention.

  • Demonstrates thorough understanding of all requirements, practice standards and regulatory requirements relating to the administration of medications, hospital policy and procedures, documentation requirements, medication education.

  • Responsible for the safe transport and administration of both controlled and non-controlled medications to patients.

  • Demonstrates thorough knowledge and therapeutic intervention skills based on knowledge of psychiatric mental health disorders; human growth and development, principles of psychiatric rehabilitation (recovery model), providing care within scope of practice.

  • Demonstrates thorough knowledge of all applicable nursing services and related policy and procedures including adequate knowledge of The Joint Commission (TJC) standards as related to position. Proper utilization of medical equipment including glucometer.

  • Continues professional development through in-service education, workshops, conferences and self-study necessary to maintain current knowledge applicable to the position. Compliance in all mandatory Human Resources requirements.

  • Is sensitive to cultural diversity issues, treats patient as an individual, and considers the culture of the patient when providing care and treatment.

  • Is knowledgeable about care and treatment needs of patients of different ages. Provides individualized care and treatment that is consistent with/sensitive to the age and life span developmental needs of each patient.

  • Must be able to apply principles of critical thinking to a variety of practical and emergent situations and accurately follow standardized procedures that may call for deviations.

  • Must be able to apply sound judgment beyond a specific set of instructions and apply knowledge to different factual situations.

  • Must be alert at all times; pay close attention to details.

  • Must be able to work under stress on a regular or continuous basis.

  • Perform other duties as assigned.

The VITAS Healthcare Telecare LPN is a member of the interdisciplinary team who is responsible for identifying the physical, psychological, social, and spiritual needs of those patients and families needing assistance during alternate hours. All clinical tasks for this supporting position will be performed under the collaboration and direction of the Telecare RN as needed. * Provide clinical assessment and intervention utilizing the nursing process, VITAS Palliative Care Guide, and triage protocols. * Coordinate all service delivery after hours including patient and family education, dispatching of interdisciplinary visits, and coordination of services with external vendors and resources as needed. * Provide support and collaboration with Telecare staff to ensure favorable patient outcomes. * Provide bereavement support to families. * Document interactions with patients, families, contacts, and vendors in the patient record to include the assessment, plan of care, caller agreement to the plan, any actions and interventions and the resolution of each patient/family interaction. * Participate in the orientation of new team members as assigned by the supervising manager. * Attend regularly scheduled in-services, staff meetings, and educational conferences. * Develop and achieve professional growth goals and objectives and reviews with supervising manager on a monthly basis. * Represent nursing and VITAS Healthcare in a positive manner. * Seek certification in the specialty of hospice nursing when qualified to do so. * Instruct and document appropriate use of medications, home medical equipment and supplies. * Appropriately utilize the resources of contract personnel such as Pharmacy, HME, agency staff, and transportation services. * Promote a customer service-oriented approach to care delivery. * Support and promote all Care Connection Center cultural platforms * Compliance with all departmental standards, policies, and procedures with training and education, management of the workload, disaster planning, attendance, adherence, and quality standards * Support the VITAS mission, core values, and Care Connection Center cultural platforms at all times.
