About this role

Our Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) is responsible for assisting in the delivery of patient care through the nursing process of assessment, planning, implementation and evaluation. Under the supervision of the RN(s), directs and guides patient teaching and activities that commensurate with his/her education and demonstrated competencies.

What you bring to the table

Education:

  • Graduation from an accredited School of Nursing

Experience:

  • One (1) or more years clinic experience preferred

Licenses/Certifications:

  • Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN)

  • Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) within the state

  • Current CPR certification

What you will do

  • Under supervision of a Registered Nurse (RN), assists in the assessment of the physical, psychological and social dimensions of patients in the Health Care Unit and, as necessary, in the housing units.

  • Assists in planning an individual treatment program by using available resources in planning care, and consults with RNs and other staff as appropriate while applying knowledge and resources in planning care and patient teaching.

  • Implements individualized treatment programs as directed by the Health Care Practitioner.

  • Count controlled substances, syringes, needles and sharps at the beginning of each shift with another staff member and sign count logs.

  • Implements clinical and technical aspects of care in accordance with established policies, procedures and protocols. Intervenes with proper safety techniques, procedures and standard precautions.

  • Responds to a code or health emergency within standard guidelines.

  • Implements medical plan through administering medications in accordance with Health Care Practitioner's orders and protocols: Administers medications according to proper techniques and procedures including IV therapy (when certified) and all other approved routes of administration. Uses pharmacy knowledge and available resources to include drug reaction and overdose in administration of medications.

  • Implements medical plans through obtaining diagnostic tests in accordance with Health Care Practitioner's orders and protocols: Obtains body fluid specimens and performs EKG's using proper techniques and procedures. Communicates information to ancillary departments using established referral process.

  • Assists the Health Care Practitioner in medical or minor surgical procedures as necessary and/or requested to meet individual needs of patients.

  • Implement nutrition and therapeutic diet plan through proper techniques and procedures as ordered by Health Care Practitioner.

  • Documents nursing encounters using the SOAP form of charting as required by policy and procedure.

  • Communicates information to nursing staff, physician, health care unit supervisory personnel and other staff as necessary.

  • Respects dignity and confidentiality of patients.

  • Attends mandatory staff meetings and training.

  • Must be able to obtain and maintain security clearance.

  • Must be able to apply principles of critical thinking to a variety of practical and emergent situations and accurately follow standardized procedures that may call for deviations.

  • Must be able to apply sound judgment beyond a specific set of instructions and apply knowledge to different factual situations.

  • Must be alert at all times; pay close attention to details.

  • Must be able to work under stress on a regular or continuous basis.

  • Post orders, if applicable, per site contract.

  • Perform other duties as assigned.

VIEW JOBS 1/22/2022 12:00:00 AM 2022-04-22T00:00 The Licensed Practical Nurse is a licensed nurse who provides competent, compassionate nursing care to patients and families under the direct guidance and supervision of a physician. The LPN reports directly to the Ambulatory Nurse Manager and the Medical Director of Phototherapy Unit. Responsibilities include the administration of UVA, UVA-1, PUVA (oral) and UVB (narrow-band), combination light treatments to patients as well as phototesting. Employee must also be able to render directed UV therapies such as Selective Ultraviolet Phototherapy, in addition to performing all tasks delegated to assist in the evaluation, management, and treatment of patients. PRINCIPAL DUTIES AND RESPONSIBILITIES: * Orients patients on phototherapy treatment and prepares a schedule of treatments. * Review patient charts and physician orders to ensure appropriate adherence to protocol. * Schedules all treatment appointments and checks in every patient for treatment in addition to collecting any copayments or out of pocket balances the patient may have. * Administers various phototherapy treatments including UVB and PUVA. Adjusts phototherapy dosing based on physician orders and response to previous treatment. * Examines patient's skin for any unusual reactions, skin irritation, reports any significant findings to physician of the day. * Appropriately documents all treatments in phototherapy database and uses electronic health record for documentation of all patient interactions. * Monitors patient's compliance with safety procedures, including need to be seen and consented by an MGH Dermatologist at least once a year, and adherence to treatment schedule to prevent adverse reaction to therapy. * Administers photodynamic treatments (PDT), including application of ALA and Blu-U light exposure. * Perform light testing, including photopatch testing, as directed by the doctor. * Answers phototherapy telephone and listens to voice mail messages at a minimum of twice daily. * Validates patient demographic information and ensures that the proper procedures are followed by directing the patient to the Registration Referral Center for registration updates. * Verifies insurance information including the coordination and tracking of managed care insurance referrals or authorizations by utilizing payer websites or calling insurances directly. * Maintains appropriate phototherapy inventories. * Updates phototherapy information sheet given to patients about schedule changes and disseminates this information to patients. * Meters phototherapy boxes biweekly to determine energy output levels. * Maintains condition of equipment used in the department, including calibration and replacement of the bulbs and calibration of meters. * Monitors the cleanliness and appearance of the Phototherapy Unit which may require assistance from Environmental Services when necessary. * Participates in meetings of clinical and administrative personnel as requested. * Provides cross-coverage for the other members of the unit in vacation. * At the time of patient appointment, brings the patient from the waiting room to the exam room and prepare the patient for the physical examination. Takes brief intake history and obtains/records vital signs when needed. * Sets-up for skin biopsies and surgical procedures, as well as, assisting with these procedures when working in General Dermatology. * Provides wound care and post-operative instructions per forms. * Performs suture removals. * Administers medications within parameters defined by the Dermatology Service after successful orientation, including the administration of biologics as specified by doctor's orders; performs nursing procedures within the parameters defined by the Dermatology Service. * Triages patient phone calls. * Inform patients of laboratory and pathology reports. * Ensures that all necessary therapies are administered as scheduled by the physician. * Provides patient education about proper use of medications. * Troubleshoot routine/non-routine problems and takes measures to correct/handle the issue.
