Community Care is hiring Licensed Practical Nurses/LPNs who are committed to teamwork and providing great care to become a part of our compassionate caregiving team. The Licensed Practical Nurse/LPN provides nursing care and services for our residents. This is a full time opportunity on our day shift for Sacred Heart location at 1555 S. Layton Blvd, Milwaukee, WI 53215.


Licensed Practical Nurse/LPN Job Responsibilities:
  • Work in collaborationwith other nursing staff to coordinate and provide direction of care during weekday operations.

  • Serves as coordinator of care for scheduled weekend and holiday shifts.

  • Ensures care and service delivery are in conformance with regulations, policies, procedures, and standards, established by Community Care, the Program for all-inclusive Care for the Elderly and the State of Wisconsin.

Licensed Practical Nurse/LPN Job Requirements:
  • Education:Diploma from accredited program in practical nursing required.

  • Licensure: A current Wisconsin Licensed Practical Nurse/LPN license in good standing

  • Certification: Current CPR/AED certification

Licensed Practical Nurse/LPN Job Schedule:

  • 6:30 AM - 3:00 PM 40 hours per week

  • Every other weekend

  • Every other holiday
