Responsibilities:

  • 1)Assumes Administrative and Skilled Nursing duties as needed on each shift.
  • 2)Works as a team with program staff toward the best interests of persons served.
  • 3)Ensures that individuals being served obtain and maintain their optimal level of health.
  • 4)Implements PCP, TAR, or IHP plans as directed.
  • 5)Implements the nursing activities of each home assigned to work in.
  • 6)Maintain the homes safety and maintenance by reporting, implementing, and monitoring for communicable disease or infections or other medical concerns; also completes vitals, body checks, administers medications as ordered or needed, etc.
  • 7)Provides direct care staff with assistance as needed and helps to maintain the cleanliness of the home.
  • 8)Documents all care daily in the required software systems. (Practical Health Systems- PHS)
  • 9)Communicates accurate information to others in a professional and courteous manner, including outside entities.
  • 10)Behavior exhibits professional attitude, demeanor, and interpersonal skills required by position and the company.
  • 11)Maintains compliance with various federal and state regulations and various agency policies/procedures.
  • 12)Other duties as assigned.

Requirements

Position Qualifications:

  • Prefer a minimum of 1-2 years experience working with the Developmentally Disabled or in a similar scope.
  • Current, valid licensure as a Licensed Practical Nurse with the state of Tennessee.
  • Knowledge of direct care nursing and documentation responsibilities.
  • Knowledge of patient care charts and patient histories.
  • The ability to evaluate therapeutic programs and services for individuals receiving nursing services.
  • Strong organizational and coordination skills.
  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community including with non-licensed medical personnel; the ability to communicate technical information to non-technical personnel.
  • Time management and follow-up skills with ability to plan, organize, and prioritize a workload; and handle several tasks simultaneously.
  • May be required to attend medical appointments with the individuals for accuracy of medical information disseminated and for safety.
  • The ability to make administrative/procedural decisions and judgments.
  • Working knowledge of computers.
  • Must be at least 21-years of age, must be able to pass thorough criminal background checks, and must pass a pre-employment drug screening.
  • Operation of a motor vehicle is required which includes: proof of a current, clean, valid Tennessee drivers license.

Physical Demands:

  • Ability to sit or stand for long periods of time.
  • Must be able to bend, stoop, run, block, and walk for long periods of time.
  • Must be able to utilize proper body mechanics while assisting with dependent transfers and positioning of individuals up to 300 pounds. This may include the use of gait belts, Hoyer lifts, and/or other mechanical lifts/methods.
  • Ability to speak and communicate effectively with others through verbal and written mediums. May include the use of assisting devices to compensate.

Benefits

Hourly rate of $20.00 daytime $21.00 nighttime.

The PRN Home Care Licensed Practical Nurse is responsible for: (1) providing direct nursing care as prescribed by the physician to acutely ill, and/or chronically ill patients in their homes; (2) recording clinical and progress notes; and (3) coordinating other patient care activities/services to provide the highest quality of patient care in the geographic service area to which she/he is assigned. She/he works under the direct supervision of the Registered Nurse and/or designated supervisor. Nursing activities are limited to these in compliance with the State LPN/LVN Practice Act, any applicable licensure/certification requirements and agency policies and procedures. (Electronic Visit Verification) program as applicable. * Prepares equipment and material for use according to the plan of care, practicing aseptic sterile technique. * Assists patient in learning appropriate self care technique. * Re-evaluates and records the patient care needs of the acutely ill and/or chronically ill at each home visit. * Communicates with and regularly updates the patient regarding his/her plan of care. * Administers medications as prescribed by a physician and permitted by law. * Adheres to minimum productivity requirements as defined by branch. * Reports observations of the patient's condition to the designated supervisor(s). If the findings are significant, report observations to the clientÂ's physician. * Assists with teaching the patient and family members regarding the patient care needs and other related problems of the patient at home. * Collaborates with other service providers to discuss the patient plan of care and frequency of visits. * Participates in providing 24-hour, seven-day-a-week nursing services as assigned, to meet identified patient/family needs. * Drives safely, complies with traffic regulations, and wears a seat belt when business travel is required. * Acts as role model in confidentiality, professionalism, appropriate attire, unbiased interpersonal relationships, and work attitudes. * Ensures confidentiality of patient records/information. * Attends team meetings and staff meetings to enhance team communication, coordination of services, and quality services. * Implements a multi-disciplinary approach to patient care by assessing and identifying the patient's need for other agency services and communicating that need to the designated Case Manager. * Participates in interdisciplinary case conferences and in service education programs, as required by agency policies and patient needs. * Participates in performance improvement activities. * Observes, records, and reports (1) signs and symptoms; (2) reactions to treatments and medications; and (3) any change in the patients physical or emotional condition in a timely matter. * Records clinical and progress notes at the time of the visit to ensure continuity of qualitative patient care in a timely basis. * Completes daily report sheets and any required paperwork in a timely fashion to ensure agency operating schedule to include physician orders as required for RN to co-sign * Completes clinical documentation accurately * Reacts to change productively and performs other job-related tasks and duties as assigned. Education & Experience Experience: 1 year Education Requirements * Must meet educational/experience requirements outlined in the laws of Tennessee. Experience Requirements * A minimum of one year of acute care experience required * Previous home care experience preferred. License Requirements * Must have current LPN licensure in state of Tennessee. * Current driver's license, vehicle insurance and access to a dependable vehicle or public transportation. * Current CPR certification required.
