The Brockton Multi Service Center Adult Community Clinical Services (ACCS) Group Living Environment Program operated by the Department of Mental Health (DMH) is seeking a dynamic candidate who will serve as a Licensed Practical Nurse (LPN II) in a residential setting. Provides behavioral and physical health monitoring and support, medication administration and education, provides trauma-informed, person centered interventions that target skill development in the areas of Health and Wellness; conducts routine assessments and provides medical care as needed; ensures that appropriate medical services are accessed in conjunction with client needs.

Position #00287656 (Whitman) - There is 1 Position Available on the 1st shift 6:45 am - 3:15 pm. The days off are Tuesday and Wednesday.
Duties and Responsibilities (these duties and responsibilities are not all inclusive
Provides primary nursing care by assessing health status, recording related health data, evaluating outcomes and adjusting care in conjunction with other service providers.Transcribes and ensures implementation of physician's orders..
Conducts self-medication readiness assessments at the time of admission and during care transitions.
Assess persons served mental status and assisting them with utilizing coping strategies, natural supports and safety planning when necessary.
Consults with ACCS Integrated team regarding client medical issues, status and serves as a liaison to PCPs, specialists, and any other involved medical providers.
Applicants must have at least 1 year full time or equivalent part-time professional experience in the field of nursing as a licensed practical nurse.
Current and valid license as a Licensed Practical Nurse in Massachusetts.
Possession of a current and valid class D Motor Vehicle Operator's license required for this function.
Ability to plan, organize, and participate in general and specialized nursing care of psychiatric patients.
Knowledge of theories and principles of nursing, including psychiatric and mental health terminology.
Knowledge of the types and characteristics of drugs used in diagnosing, treating and controlling physical, mental and/or emotional disorders
Experience working with individuals with serious, persistent mental illness, substance abuse and/or who have experienced homelessness.
Previous experience working in a residential setting
Given the population served, bilingual or multilingual fluency.

MINIMUM ENTRANCE REQUIREMENTS
Applicants must have at least one year of paraprofessional experience in the field of nursing as a licensed practical nurse.
SPECIAL REQUIREMENTS: Possession of a current and valid license as a practical nurse under the Massachusetts Board of Registration in Nursing.
