The Brockton Multi Service Center Adult Community Clinical Services (ACCS) Group Living Environment Program operated by the Department of Mental Health (DMH) is seeking a dynamic candidate who will serve as a Licensed Practical Nurse (LPN II) in a residential setting. Provides behavioral and physical health monitoring and support, medication administration and education, provides trauma-informed, person centered interventions that target skill development in the areas of Health and Wellness; conducts routine assessments and provides medical care as needed; ensures that appropriate medical services are accessed in conjunction with client needs.

Please Note: "This is a Seasonal position for an anticipated duration of 90 days"

Position #00287656 (Whitman) - There is 1 Position Available on the 1st shift 6:45 am - 3:15 pm. The days off are Tuesday and Wednesday.

Duties and Responsibilities (these duties and responsibilities are not all inclusive

  • Provides primary nursing care by assessing health status, recording related health data, evaluating outcomes and adjusting care in conjunction with other service providers.Transcribes and ensures implementation of physician's orders..

  • Conducts self-medication readiness assessments at the time of admission and during care transitions.

  • Assess persons served mental status and assisting them with utilizing coping strategies, natural supports and safety planning when necessary.

  • Consults with ACCS Integrated team regarding client medical issues, status and serves as a liaison to PCPs, specialists, and any other involved medical providers.

Required Qualifications:

  • Applicants must have at least 1 year full time or equivalent part-time professional experience in the field of nursing as a licensed practical nurse.

  • Current and valid license as a Licensed Practical Nurse in Massachusetts.

  • Possession of a current and valid class D Motor Vehicle Operator's license required for this function.

Preferred Qualifications:

  • Ability to plan, organize, and participate in general and specialized nursing care of psychiatric patients.

  • Knowledge of theories and principles of nursing, including psychiatric and mental health terminology.

  • Knowledge of the types and characteristics of drugs used in diagnosing, treating and controlling physical, mental and/or emotional disorders

  • Experience working with individuals with serious, persistent mental illness, substance abuse and/or who have experienced homelessness.

  • Previous experience working in a residential setting

  • Given the population served, bilingual or multilingual fluency.
