Bel Air Health and Rehabilitation Center is a 155 bed, Skilled Nursing Facility offering a combination of short term rehab and long term care services. We are seeking passionate team players to help us in our mission: To enhance and inspire the lives of all we serve: our team members, our patients, our residents and our families.

Offering a $3,000 Sign on Bonus.

Always accepting applications for team players to be considered for 1st, 2nd and 3rd shifts and weekends.

In this role, you will provide direct patient care within a skilled nursing facility. You'll follow the plan of care, assessing patients, administering medications and treatments, preparing supplies and equipment for treatments, and assisting the attending physician or registered nurse in performing specialized procedures.

Job Requirements

  • Graduate of an accredited school of Nursing.

  • Valid LPN License.

  • Current CPR certification.

VIEW JOBS 11/2/2018 12:00:00 AM 2019-01-31T00:00 JOB SUMMARY: Works collaboratively with inter-disciplinary team to assess plan of care, monitor progress and make recommendations so the safest and most efficient hospitalization is achieved. Responsible for evaluating patients accessing care through ED for medical necessity and care coordination needs. Assesses patients who may be admitted and collaborates with advanced practice clinicians/physicians to ensure the appropriate admission status is used. Proposes potential outpatient plans that would safely allow patients to be discharged from the ED and implements with physician approval. Coordinates abuse, chronic pain and frequent utilization population. Orients new team members. May interact with patients or customers ranging in age from newborn to geriatric. MINIMUM KNOWLEDGE, SKILLS AND ABILITIES: Education & Training: Current Maryland RN or Master's in Social Work license (LMSW/LCSW) required. Completion of a Bachelor's of Science degree in Nursing preferred. Work Orientation & Experience: Minimum of 3 years acute care experience required, 5 years for Social Workers. Basic computer skills required. ED experience preferred. Experience in utilization management, case management and discharge planning preferred. Skills & Abilities: Demonstrate skill in a) clinical case management; b) performing complete assessments; c) effective critical thinking skills both written and oral; and d) age appropriate interpersonal interactions (patients may range from newborn to geriatric adult.) Ability to a) communicate and collaborate effectively with both internal and external customers (colleagues, Medical Staff, liaisons, and patient/family); b) assess, adapt, and calmly respond to changing and/or crisis environment; c) make independent decisions consistent with current policies, procedures, and ethical standards; d) prioritize work assignments and manage time effectively to complete duties; and e) assist in data analysis.
