Position Overview:

• Provides direct patient care utilizing the nursing process under the direction and supervision of the Registered Nurse.

• Reviews diagnostic test results and reports abnormal results and changes in patient condition to the appropriate healthcare team members.

• Collaborates with the Registered Nurse on development and implementation of the patients plan of care.

• Administers prescribed medications, therapies and treatments in accordance with Licensed Practical Nursing Standards.

• Maintains and reviews patient medical records, notes, and physician orders. Documents all patient care activities.

• Assists physicians, Registered Nurse and multidisciplinary team with data collection, planning, intervention and evaluation of patient and family care to achieve desired outcomes.

• Implements components of the plan of care with the Licensed Practical Nurse scope of practice and competency to promote desired outcomes.

• Assists with discharge planning and carrying out the patients teaching plan under the direction of a Registered Nurse.
