CareHarmonys Licensed Practical Nurses are experts in helping patients and caregivers navigate the healthcare system and better manage the patients chronic illnesses. LPN Care Coordinators work cooperatively with the patients providers to help achieve health goals.

Position Details This is a telephonic coordinator position with no face-to-face patient contact. CareHarmony is seeking a Licensed Practical Nurse, with at least one year of prior clinical work experience, to support providers in delivering comprehensive chronic care management services for their patients.

All work will be Remote. You must have internet, preferably Fiber or Cable, in order to be considered for an interview. Applicants will not proceed to the interview stage without proof of a reliable home network.

Each LPN Care Coordinator is assigned a set census of patients. You will be responsible for providing all telephonic support required to coordinate care for their patients. You have experience identifying and coordinating the needs of chronically ill patients and families as well as supporting clinicians to help them operate as efficiently as possible. You also have the proven ability to critically analyze and use nursing judgement with complex patients.

Responsibilities

  • Maintain and monitor your own patient census.
  • Conduct telephonic care coordination with chronically ill patients and their families.
  • Assess, identify, and close clinical and non-clinical gaps in patient care.
  • Manage referrals and appointment scheduling.
  • Identify community resources (both clinical and non-clinical) that would be beneficial to your patients and help them access those resources.
  • Provide patient education on management of chronic conditions or on post-discharge instructions.
  • Perform medication management, including identifying potential medication issues, answering patient questions on side effects and medication conflicts, and coordinating refills.
  • Assist in ensuring timely delivery of services to your patients such Home Health, DME, Home Infusion and other critical needs.

Requirements

  • Home internet that meets company requirements
  • Strong computer skills
  • Current Multistate/Compact license required
  • Two years of experience required
  • Prior healthcare experience serving chronically ill patients preferred
  • Basic technology skills required should be very comfortable with computers
  • Telephonic case management or care coordination experience preferred
  • Strong customer-service orientation
  • Compassionate and friendly demeanor
  • Enjoys working in a fast-growing, rapidly changing environment
  • Bilingual skills a major plus
  • Full time- set schedule (not flex)

We expect our nurses to thrive in a fast-paced environment and always put the patient first.</p><p>We are currently looking for compassionate and motivated <strong>bilingual Spanish-speaking RN’s</strong> who possess a solid background of clinical nursing skills, technological competence, ability to multitask and think critically while speaking on the phone with hospice patients, families, or client facilities. Our nurses must have at least three years of clinical experience (preferably home health or hospice) – and a compact nursing license is strongly preferred. Our nurses are also very skilled using technology and technology platforms to perform nursing tasks.</p><p>As a triage nurse, <strong>we can offer both full-time and part-time positions </strong>depending on needs of the business at the time with a minimum expectation of 16 hours per week. Additionally, all of our triage nurses work every other weekend. We offer a wide variety of shift types, offering shifts as short as 4 hours or as long as 10 hours – put those long days on the floor and in the car behind you! These shifts are primarily in the evenings and weekends.</p><p>Our nurses enjoy working from their own home office and are required to provide their own high-speed internet (fiberoptic or coaxial cable). We will provide you with a computer and headset. It is also essential to have a home office or quiet space free from distractions in your home. Training is provided and paid, and no hands-on care or travel is required. All nurses enjoy the benefits of working from home, providing care, without the stress and expense of commuting each day.</p><p>This role reports to the Chief Nursing Officer.</p><p><strong>Requirements</strong></p><ul> <li><strong>Must be fluent in Spanish and in English. </strong></li> <li>Must have RN license; compact state license is strongly preferred.</li> <li>Must be comfortable with technology and accessing EMRs remotely to perform documentation.</li> <li>Must be able to complete three-week orientation (to include first week daytime hours).</li> <li>Must be available to work every other weekend and holidays as required.</li> <li>Must be able to work 16+ hours weekly for part-time and 30 hours+ for full-time.</li> <li>Able to handle stress and multitask when calls are coming in.</li> <li>Able to communicate with patients and families with empathy while also maintaining adherence to client protocols.</li> <li>Must maintain CEU’s as designated by the states you are answering calls in.</li> <li>Must attend any in-services, additional training on a as needed basis.</li> <li>Able to pass background check and nurse licensing check.</li> <li>3+ years of clinical nursing experience required and 1+ years of hospice care experience.</li> </ul><p><strong>Benefits</strong></p><p>Compensation is between $23-$25 per hour depending on hospice and telephonic triage experience. Nurses will be provided their own computer and headset. Nurses will be reimbursed for any licensing costs outside of their home state.
