Requirements for LPN Transition to Practice Program

  • Graduation from an accredited nursing program (must graduate before program starts in September)

  • LPN Certificate

  • Current Massachusetts Nursing License (must have license before program starts in September)

  • Less than one year of ambulatory care nursing experience

Program Summary: This position is a time limited educational experience to introduce the new graduate LPN to nursing practice from multiple perspectives through didactic, clinical and experiential opportunities. Participates in all didactic sessions as part of the role, as well as learning to participate in the planning, delivery and management of direct patient care utilizing the nursing process and adhering to the standards of nursing practice.

Program Description: The LPN Transition to Practice program provides recent graduates of nursing schools with an evidence-based curriculum of didactic and clinical skills to support the transition to Ambulatory practice. During the program, participants will attend classes, complete patient care with an experienced preceptor, and participate in simulation to further develop critical thinking skills and increase confidence in safe delivery of care.

Program Outcomes: Enhanced ability to provide quality, evidence-based care, improved skill in clinical decision making and critical thinking.

Variety of shifts available, Day shift some clinics might have late evening or Saturday hours. Most locations are located at our University or Memorial campuses. Some openings in other areas (Leominster/Marlborough)

I. Major Responsibilities:

1.Assesses and obtain a patients basic health status, records, and related data according to standard procedure and needs of patient and their families.

2.Administers medications, performs treatment as ordered, and operates equipment according to policy and procedure.

3.Performs venipuncture using aseptic technique, 12 lead EKGs, and administers vaccinations.

4.Participates in implementation and evaluation of care plan including long and short-term goals, nursing actions, and teaching.

5.Documents patient care according to standard procedure. Records and reports observed symptoms, reactions, treatments, and changes in patient condition to Provider and/or RN and assists in analyzing and interpreting patient data.

II. Position Qualifications:

License/Certification/Education:

Required:

1.Completion of an accredited practical nursing program.

2.Current Massachusetts licensure required.

3.Basic Life Support (BLS) certification.

Experience/Skills:

Required:

1.Knowledge of basic nursing procedures.

2.Strong interpersonal skills are required to gather information from individuals, build relationships that support collaborative work within the department and across the organization and provide exceptional service to patients.
