LPN Summary:

Our LPNs provide resident care by utilizing the nursing process in assessment, planning and implementation of care. They exhibit organizational abilities related to work flow and prioritizing resident care needs. They use current standards of practice and follow current facility policy and procedures while providing resident care. They support the mission of The Health Center at Standifer Place and exemplifies The Better Way through leadership and demeanor. They represent the center in a positive manner both in the medical community and in the community as a whole.

Education and Training:

  • Graduate from a nursing program accredited by the State.

  • Maintains a current unencumbered licensed to practice in the state where employed

  • Seeks opportunities for personal and professional growth.

  • Attends continuing education offerings and through teaching and mentoring, contributes to the ongoing education of partners under his/her supervision

Job Knowledge and Capabilities:

  • Our LPNs are responsible for maintaining clinical competency as evidenced by application of integrated nursing knowledge and skills, leadership and communication skills.

  • Utilize the nursing process in assessment, planning and implementing care.

  • Exhibit organizational ability related to work flow, by prioritizing to meet the patient's care need.

  • Integrate current standards of practice as well as local, state, and federal regulations related to nursing services in the care of patients.

  • Practice continuous quality improvement thinking and problem solving skills

Specific Duties and Responsibilities:

  • Working with the interdisciplinary care team, to assure accurate patient assessment and to develop/revise individualized plans of care.

  • Maintains open and ongoing communication with patients and families.

  • Provide opportunity and encouragement for patients and their families to participate in decision making.

  • Assure that nursing partners have been assigned duties

  • Make rounds, with specific attention to high acuity patients

  • Monitor unit/units to ensure that appropriate nursing care (according to established policies and procedures) is being provided and that the medical staff and families are being notified of changes in patients condition

  • Assume responsibility for assisting with medications and treatments as needed.

  • Assist with admitting patients and processing of medical staff orders as required.

  • Assist with the supervision, and orientation of new nursing partners

  • Assures the personal dignity and physical safety of each patient. Assist patients to attain the highest level of self-care possible.

  • Supervise nursing partners as assigned.

  • Carry out personnel disciplinary action(s) promptly and effectively as stated in personnel policies and amendments.

  • Maintains the security of the premises and direct unauthorized persons coming on the premises to leave. Calls security for assistance as needed.

  • Performs additional duties as assigned.

  • Relieve other shifts if needed

  • Ability and willingness to be assigned work on different units according to staffing needs

  • Ability to work in settings were patients' needs, actions, and responses to others are widely varied, where unpredictable reactions and/or emergencies occur, and where your immediate, professional and compassionate action is required.

This position is responsible for the health needs of the residents and maintaining the well-being and overall experience of the residents. Essential Duties and Responsibilities include the following: * Directly supports the Director of Nursing by providing on-call relief. * Oversees and ensures the delivery of quality care and services to our residents in compliance with the regulations pertaining to care plans and resident assessments. * Ensures the delivery of care to residents to ensure that emotional, physical, psychological and safety needs are met through all program services and activities. * Ensures infection control, pharmaceutical and physical/chemical restraining policies and procedures are followed. * Administers medications and/or treatments for all or a selected group of residents, adhering to facility policies and procedures. * Strives to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices. * Oversees and assures completion of any required documentation in the resident's medical record. * Completes accident and incident reports, and monthly infection control logs according to facility policy. * Maintains current knowledge of scope of practice for self and others and recognize legal implications when making nursing judgments. * Informs the Director of Nursing, or other appropriate staff regarding changes in resident physical, emotional or behavioral status, orally and via the Stop and Watch tool. * Initiates and contributes to the plan of care for residents, according to resident needs and policy, to promote continuity of care from admission through discharge or transfer. * In the absence of the Director of Nursing the Nurse will be responsible for the overall nursing operations of the facility. * Other duties may be assigned. Requirements Education * Bachelor Degree and 0-2 years of experience OR * Associate degree and 2-4 years of experience * Accredited LPN or RN degree * Applicable state licensure, required Skills/Experience * One to two (1 - 2) years of nursing experience in a senior care community with proven leadership and organizational skills * Demonstrated compliance with State and Local regulations as well as professional standards and practices. * Collaborative team player able to establish effective working relationships with team members. * Demonstrated experience in promoting a high level of customer service and hospitality towards and among all residents, associates, and visitors. * Self-motivated to accomplish goals with a strong sense of accountability for results. * Demonstrated communication excellence, verbally and in writing; comfortable presenting to large groups. * Possess an appreciation of seniors and an understanding of the aging process. * Must possess a general knowledge of laws and restrictions regarding Assisted Living, Memory Care and Independent Living (where applicable), as well as a solid understanding of opportunities and limits of other levels of care within senior living. * Must exhibit and promote a high level of customer service, hospitality, curiosity and friendliness towards all prospects, residents, visitors, employees and the overall community.
