Job Summary

Job Summary

Deliver licensed practical nursing (LPN) care within the LPN scope of practice, as defined by the Minnesota Nurse Practice Act, for cognitively and physically disabled residents. Work collaboratively with an interdisciplinary team to promote the well being, health, and safety of residents in a person-centered, long-term care environment.

This is an intermittent position. An intermittent employee works an irregular and uncertain schedule as the needs of the agency require. Intermittent employees must be available to work all three shifts and some weekends.

This position is not initially eligible for health care benefits.


Minimum Qualifications:

  • Licensed in Minnesota as a Practical Nurse with no restrictions

  • Ability to communicate effectively, follow instructions, and take clear notes

  • Current Basic Life Support (BLS) or Certified Pulmonary Resuscitation (CPR)

Preferred Qualifications:

  • Experience working in a long term care environment.

  • Experience working with Veterans

Physical Requirements

Perform a variety of movements throughout the day such as walking, standing, sitting, bending,twisting, stooping, lifting, carrying, reaching, grasping, pushing, and pulling.

Additional Requirements

  • A Minnesota Department of Human Services (DHS) background check will be conducted on finalists for this position. Finalists must pass the DHS background check.

  • A Tuberculosis (Mantoux) screening (or chest x-ray) is required prior to employment.

  • Employment reference checks will be conducted on finalists. This may include a review of documentation related to job performance and contact with the applicant's former employer(s).

Demonstrates knowledge of nursing principles for the patient in the relevant setting and possesses the ability to perform testing to assess data reflective of the patient's status and to interpret appropriately the information needed to identify each patient's requirements relative to the age-specific needs, and to provide the care needed. Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in the home care setting. Ensures the health, comfort and safety of patients. Knowledge of the principles and skills needed for nursing to provide patient care and treatment. Knowledge of medications and their effects on patients. Knowledge of patient education principles to develop a self-care program. Contribute to the evaluation of the plan of care by gathering, observing, recording and communication patient responses to nursing interventions. Establishes and maintains productive working relationships within and outside of own area. Follows safety measures to ensure proper use of equipment and supplies. Implement interventions that are assigned, ordered or prescribed by licensed practitioner or registered nurse. Observe patients, document and report changes in patient condition, such as adverse reactions to medication or treatment, and take any necessary action. Participate and be part of a healthcare team to conduct focused nursing assessments and contribute to the plan of care. Participate in health teaching developed by a licensed practitioner or registered nurse. Promote a safe and therapeutic environment by providing appropriate monitoring and surveillance of the care environment. DEPARTMENT DETAILS Required to work with patients and families at end of life. Must be able to multi-task and complete the duties of medication administration to hospice patients, prepare and serve meals, complete all daily cottag cleaning requirements, complete laundry tasks and to be able to work well with others. QUALIFICATIONS Currently holds an unencumbered Licensed Practical Nurse (LPN) license with State Nursing board and/or possess multi-state licensure privileges.
