The GuideMed Licensed Practical Nurse performs all activities included in the GuideMed Management Program. These activities include collecting risk assessment data, review of controlled substance agreements, medication reconciliations, collection of toxicology specimens, and prescription registry checks by following established network standards.
Manages the patients that come to the GuideMed service center and all activities performed inside the center.
Enters all necessary information into the GuideMed software program.
Obtains necessary patient information from the client's health records to appropriately report and track GuideMed monitoring activities.
Follows the established treatment and testing protocols in place for the client.
Reviews controlled substance agreements with patients per the client's frequency protocols and documents the review and signature of the agreement in the patient's record.
Administers patient risk assessments per client protocol under physician direction to all necessary patients and documents the responses and total risk assessment scores in the patient record.
Checks the prescription registry for patients per the client's frequency protocols, investigates findings, and document the results in the patient's chart.
Collects toxicology specimen as ordered by the client, assembles samples for shipping; completes associated paperwork accurately; collects and records patient billing information.
Ensures timely response for all toxicology affidavits or edit notifications and documents toxicology results in the GuideMed software platform.
Performs medication reconciliations per the process outlined by AIT and records all of the results in the GuideMed software.
Assists with implementing individualized patient compliance monitoring plan of care per client's treatment protocol recommendations in a timely manner along with a summary of activities performed and the results of these activities.
Requires the ability to compare data and information to parameters assigned by the GuideMed software program and communicate variation to the physician.
Requires sufficient knowledge to observe and collect data for the assessment of a patient's condition in order to detect and report any dangerous changes in the patient's condition.
Provides appropriate patient education per physician's or RN's direction, documents it in the GuideMed software program, and completes associated billing forms.
Maintains communication with supervisors concerning client questions or any other AIT related business, excluding the use of PHI (Protected Health Information).
Other duties as assigned.
High school diploma or general education degree (GED) with completion of an accredited school of nursing program, e.g. Licensed Practical Nurse.
At least 1 year experience in medical office preferred, pain management experience beneficial Current, unencumbered licensure as a Licensed Practical Nurse (LPN) in state(s) where work is being performed American Heart Association Basic Life Support (BLS) certification (Healthcare Provider certification)
Valid driver's license
Physician/clinical office or the independent GuideMed service center
May come in contact with biological specimens
Requires personal protective equipment (PPE)
HOURS OF OPERATION/SCHEDULE:
7:00 am to 6:00pm Monday through Friday
