The LPN Licensed Practical Nurse is responsible for supervising nursing personnel to deliver nursing care and within scope of practice coordinates care delivery, which will ensure that patient's needs are met in accordance with professional standards of practice through physician orders, center policies and procedures, and federal, state and local guidelines. LPN Licensed Practical Nurse Responsibilities:

This LPN position is a staff position that has direct care responsibilities as well as supervisor responsibility for nursing assistants.

Administer medications and treatments timely and according to policy

Receive, transcribe, and carry out physician orders

Follows established procedure for charting and reporting all reports of incident/accidents (patients & employees)

Identifies and reports changes in condition to supervisor, physician and family

The Licensed Practical Nurse works as a member of the multidisciplinary team to assure high quality nursing service in accordance with the physician's orders and the agency's programs and policies. Services are provided in the patient's place of residence and/or inpatient unit. JOB DUTIES AND RESPONSIBILITIES: * Collaborates with members of an interdisciplinary team to assure appropriate patient care and care planning. May communicate with the RN, therapists, physician and/or hospice medical director, orally and in writing as required by the patient's condition and agency policy. * Provides ongoing patient observation to determine service frequency and change in Plan of Care in compliance with agency policy and philosophy. * Provides comprehensive nursing care, health teaching, guidance and supervision to the individual and family at direction of RN, physician, or Medical Director. Considers psychosocial, environmental, spiritual and financial needs. * Supervises patient care given by Home Health and Hospice Aides. * The Home Health LPN promotes and maintains health of individuals, families, and the community through teaching, counseling, and appropriate preventive and rehabilitative measures in the practice of nursing. * The Hospice LPN cooperates with the Volunteer Coordinator and is available to hospice volunteers for advice and training when dealing with the patients and families. * The Hospice LPN participates in the bereavement program and supervises volunteers in providing bereavement care. * In consultation with Patient Care Manager, refers patients to other disciplines and to community resources as needed. * Maintains and uses patient clinical records, reports and other written communications according to agency policy and directives. Records make clear the goals and plans with emphasis on the family-oriented approach to patient care. Records reflect contact with physicians, Patient Care Manager and other agencies as needed. Documentation is completed per agency guidelines. * Participates in the development and operation of the agency by: * Keeping administration informed on changing community needs and lack of community resources. * Participating in the orientation and guidance of new staff. * Interpreting agency service to families and community groups. * Contributing to evaluation of service programs. * Evaluation effectiveness of his/her own service and contributing to the evaluation of and continuing development of staff members. * Participating in the development of the in-service programs and group teaching for staff and other groups, as assigned. PHYSICAL AND SENSORY REQUIREMENTS: Physical Demands: Heavy Work Exertional activity: Occasionally lift and/or carry 100 lbs; Frequently lift and/or carry 50 lbs; Stand and/or walk at least 6 hours in an 8-hour work day. Nonexertional activity: Stooping (bending at the waist) and crouching (stooping and bending at the knees) - required frequently; Handling (grasping) - required frequently; Fingering (fine manipulation) - required only occasionally. Important: The functional capacity to perform heavy work includes the functional capacity to perform medium, light and sedentary work. EDUCATION: Graduation from a School of Practical Nursing with a current license as a Practical Nurse. TRAINING AND EXPERIENCE: A minimum of two years experience as an LPN.
