Phoenix House is looking for a Full-Time LPN for our Ronkonkoma location. The LPN will provide prescribed treatment and nursing care to patients. Implement doctor's orders, monitor specified medication/refills. Monitor and record vital signs as needed, and maintain medical/nursing records.

Responsibilities:

  • Administer prescribed medications; monitor vital signs and other detoxification symptoms/responses.

  • Coordinate provision of medications with providers and pharmacy

  • Maintain medical/nursing records.

Skills:

  • Crisis management and be organized.

  • Computer skills

Educational/Credentials:

  • Licensed Practical Nurse for New York State

  • NARCAN/Opioid Overdose Reversal

  • CPR/First Aid

Experience:

  • Administration of medications, acquisition of vital signs and documentation of such interventions.

  • Ability to communicate with a multi-disciplinary team

  • Experience with substance abuse patients and the ability to identify signs/symptoms of overdose and withdrawal.

RESPONSIBILITIES: + Facilitate groups on a daily basis while maintaining a caseload of individual clients and documenting in electronic health records in a timely fashion. + Maintain current, accurate and confidential clinical charts. + Provide individual therapy, treatment planning, case management, client advocacy, group counseling, and crisis intervention as needed. + Models and provides pro-social behavioral interventions. + Participates in the treatment planning process and remain aware of treatment goals of resident. + Facilitate interactions with residents within the milieu to build social, emotional, and behavioral functioning. + Assist in the assessment of independent living and recovery skills. + Ensures that clients adhere to the day-to-day program schedule, tracking attendance as required. EXPERIENCE: + Experience working with chemical dependency patients in a residential setting and with electronic medical records. KNOWLEDGE/SKILLS: + Organized, responsible, dependable, sound decision making, professional boundaries and crisis management + Experience with co-occurring disorders and medication management a plus. EDUCATION: + Licensed Mental Health Counselor, Licensed Master Social Worker + Licensed Clinical Social Worker, preferred Phoenix House conducts, background checks, fingerprints, drug testing, TB testing, verification of employment history and/or reference checks prior to employment.
