Licensed Practical Nurse (Lpn) - Part Time - Home Health - Personal Care

Job Description Job Description Care Advantage Inc, Voted 1 in Home Healthcare the last 5 years is looking for a Licensed Practical Nurse (LPN) to join our Personal Care nursing team in a Part-Time capacity . Our team provides 1 on 1 care in patients' homes with extensive services to provide our patients with the best possible care. Our Licensed Practical Nurses assist with supervisory visits and follow-up visits based off the plan of care for patients located within the Newport News, Chesapeake and Portsmouth VA area as well as some of the surrounding areas. The Licensed Practical Nurse will effectively interact with patients, caregivers, family members and other interdisciplinary co-workers to provide nursing care to patients on an intermittent basis.

A DAY IN THE LIFE OF A Licensed Practical Nurse Makes contact with the patients and explains services offered Completes follow-up visits as needed Instructs assigned care team members on the needs and care plan details Reports progress of care to involved parties Follows up and maintains relationships with clients, always advocating on their behalf Establishes relationships with community partners and referral sources Keeps Clinical Leadership and Office Administrator abreast of patient condition and/or medical changes Reviews caregiver documentation Takes part in interdisciplinary team meetings to ensure needs of all patients are met Care Advantage Inc. is fully committed to enhancing and restoring the lives of our patients in the communities we serve. Required Qualifications: Graduate of an approved school of professional nursing Current and valid Virginia LPN License Two (2) years' Personal Care home health nursing experience, preferred.

Previous experience completing Supervisory visits, strongly preferred.

Passionate and motivated to provide care within a patient's home. Ability to exercise initiative and independent judgment. Comfortable working collaboratively in cross-functional teams Ability to communicate effectively, both verbally and in writing, with all levels of an organization Ability and willingness to travel within a given territory.
