SUMMARY
Provides nursing care and coordinates medical/psychiatric care for Red Rock clients. Provides clients with continuity of care. IF PACT: Organizes and manages the system of getting medications ordered and delivered to the clients; assesses physical health needs; makes appropriate referrals to community physicians; provides management and administration of medication in conjunction with the psychiatrist; provides a range of treatment, rehabilitation and support services and shares shift-management responsibility with other staff.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Performs nursing assessments in order to identify client's needs, plan of care. Provides treatment for existing and emergency health care conditions. Assesses for medical appropriateness for admission and continually assesses throughout clients' stay. Consults with Psychiatrist if change in medical status occurs with referral to more appropriate level of care from crisis center
Administers medication and maintains clinical charts. Observes, records, and initiates appropriate action concerning client reaction to medication and therapeutic procedures
Monitors activities and job duties of Mental Health Assistants assigned to their shifts
Reviews and monitors client's progress through supervision and disposition of cases; confers with other programs/agencies, doctors, etc. as required regarding client welfare
Documents care according to nursing standards of care, agency policy and procedures
Assists in assessment of clients' mental status on a daily basis
Performs a variety of professional medical nursing duties under the direction and supervision of the Psychiatrist; collects of routine laboratory specimens, administrates of injectable medications and emergency medication, communicates with pharmacies regarding doctor's orders for medications and related duties
Maintains core competencies in relation to working with co-occurring disorders through continuing education and implementing skills into all aspects of treatment
Counts and documents locked PRN medications every shift with guidance/supervision of registered nurse on duty
Attends staff meetings, workshops, and seminars to learn agency policy, rules, regulations, and procedures; participates in ongoing in-service training as well as pertinent external training
Assists in the development and implementation of programmatic goals and objectives
Supports and assists with the facilitation of Care Plans for consumers within the Red Rock Health Team
Performs other duties as assigned
IF PACT:
Consults with community agencies and families to maintain coordination in the treatment process
Effectively performs shift management in coordination with other PACT shift managers according to established policy and procedures
Provides on-call crisis intervention covering nighttime hours and serves as backup to evening and weekend staff
Documents client progress to maintain a permanent record of client activity according to established methods and procedures
Participates in daily staff organizational meetings and treatment planning review meetings
Takes the lead role or participates in providing medication administration and medical services
Under the direction of the team Psychiatrist and/or in collaboration with other registered nurses on the team:
Develops, revises, maintains and supervises team psychopharmacologic and medical treatment and medication policies and procedures including transcribing, administering, evaluating and recording psychotropic medications prescribed by the team psychiatrist; evaluates and charts psychotropic medication effectiveness, complications and side effects; arranges for required lab work according to protocol
Organizes and manages the system of getting medication to clients and integrating medication administration tightly into clients' individual treatment plans
Manages pharmaceuticals and medical supplies efficiently
Coordinates, schedules and administers medical assessments of client physical health, making appropriate referrals to community physicians for further assessment and treatment. Coordinates psychiatric treatment with medical treatment
Participates in treatment, rehabilitation and support services
Assists and supports clients to have and effectively use a personal physician and dentist
Attends staff meetings, workshops, and seminars to learn agency policy, rules, regulations, and procedures; participates in on-going in-service training as well as required external training
Exhibits and maintains core competencies in relation to working with co-occurring disorders through continuing education and implementing skills into all aspects of treatment
Effectively assists in the development and implementation of programmatic goals and objectives
Demonstrates competency in working with individuals with Severe Mental Illness (SMI) and chronic health conditions
Reads the PACT Behavioral Health Homes (BHH) rules and understand how mental illness affects the lifespan
Provides comprehensive case management to ensure follow-up with primary care
Provides health education specific to a member's chronic condition
Develops self-management plans with the individual
Provides support to improve social networks Promotes healthy lifestyle interventions (such as substance abuse prevention, smoking prevention and cessation, nutritional counseling, obesity reduction, and increasing physical activity)Assists clients in understanding and self-managing chronic health conditions
Supports and assists with the facilitation of Care Plans for consumers within the Red Rock Health Team
Performs other duties as assigned
QUALIFICATIONS
Registered LPN in state of Oklahoma
Demonstrated interpersonal and communication skills
Demonstrated experience in use of physical assessment skills and the ability to handle medical emergencies
Must be able to work shift-work
Commitment to the mission of Red Rock BHS
One-year experience preferred Knowledge and experience working with administration of psychotropic medications preferred
Knowledge of psychiatric nursing, practices, techniques and procedures preferred
Experience working in a psychiatric setting preferred
Certification in mental health nursing preferred
IF PACT
Registered LPN in the state of Oklahoma
At least one year of mental health experience or work a total of 40 hours at a psychiatric medication clinic within the first three month of employment
Valid Oklahoma Driver's License, clean driving record, transportation
Excellent verbal and written communication skills
General skills in exercising independent judgment, tact, patience, and understanding; maintaining effective working relationships with clients, other staff, and the community
Two years relevant experience preferred Commitment to the mission of Red Rock BHS
