Definition:

The Licensed Practical Nurse (LPN) provides skilled nursing services under the direction of the Registered Nurse.

Line of Authority:

Registered Nurse, Director of Nursing, Administrator/Director of Services; Regional Administrator; Director of Clinical Services, Vice President, HomeCare

Qualifications:

  • Licensed Practical Nurse in the state where HomeCare agency is located

  • Graduate of a state approved vocational education program

  • One year clinical experience is preferred.

Performance Requirements:

  • Ability to read, write, learn, absorb, and follow written orders and keep records in prescribed manner.

  • Ability to be reaching, bending, squatting and twisting numerous times daily.

  • Ability to lift 70-80 pounds on occasional basis.

  • Ability to see and hear adequately in order to respond to auditory and visual requests from patients.

  • Ability to speak in clear, concise voice in order to communicate adequately.

  • Ability to carry out fine motor skills with manual dexterity.

  • Able to perform routine nursing procedures.

  • Mental acuity high enough to adequately perform job requirements.

  • Must have reliable personal transportation and be able to drive in inclement weather and variable road conditions.

Specific Responsibilities:

  • Provides skilled nursing services as dictated by the plan of care and documents according to policy/procedures.

  • Instructs the patient/caregiver in various aspects of patient care and disease management. Documents the patients/caregivers comprehension of and compliance with teaching.

  • Observes patient response to care/teaching and makes recommendations regarding revision of plan for patient care to the Registered Nurse.

  • Provides and documents skilled nursing care according to the plan of care/physicians orders, policies/procedures and standards of care.

  • Records and reports to the Registered Nurse symptoms, vital signs and other observations that may be indicative of care plan reactions and changes.

  • Follows and teaches infection control and safety policies and procedures.

  • Participates in on-going care coordination and maintains positive relationships with referral sources, physicians/other clinicians, patients/families, and other partners.

  • Participates with other nurses in taking weekend, night and holiday calls on a rotating basis, under the direction of a Registered Nurse.

  • Continually strives to improve nursing care through participation in inservice activities, staff development activities and continuing learning.

  • Serves on committees as assigned.

  • Performs other duties as assigned by the RN / DON

RNs in this role work autonomously alongside the hospital staff to assess, troubleshoot, and make sound decisions for the critically ill patients. If you haven't considered Nephrology nursing before, read on as we think that you should. Dialysis Experience is NOT required. What you can expect: * 1:1 Patient Care. Deliver acute hemodialysis to in-patients with chronic kidney disease, as well as patients with acute kidney injury (AKI) and transplant. Average Patient to Nurse Ratio is 2:1 or 1:1. * Patient education. Whether this is the first or a blip in their chronic treatment, our RNs are experts in educating our patients and families. * Specialized, complex care. RNs deliver care to patients who are often dealing with multiple co-morbidities and/or acute conditions which require unique treatment plans and the ability to leverage a broad range of nursing skills and knowledge. * Not just hemodialysis. Acute RNs may deliver nephrology care across multiple renal therapies: peritoneal, continuous renal replacement and apheresis. * Expect the unexpected. When you walk in the door each day you don't know what patients you might have, their condition, when you will get them or when you will be done for the day. * Autonomy in a Hospital Setting. Deliver dialysis to patients at a particular hospital or group of hospitals and become an integral part of their team and ours. As a teammate in an acute setting you work on your own almost all of the time. Each day you assess, troubleshoot, and make sound decisions for critical patients. Our Hospital Services group performs 1.1M treatments annually. * Schedule. No two days are ever the same for an acute dialysis nurse - long days and on call are required. Schedules vary by hospital system. For this role expect to know your weekly schedule in advance, but your daily schedule may change depending on patient needs. Must be in commutable distance based on on-call requirements. * The Gold Seal.
