Mentis Neuro Health is seeking a LPN/LVN to provide prescribed medical treatment and personal services under the direction of a Registered Nurse (RN) or physician.

Qualifications:

Requirements:

  • Two year college degree in nursing or related field or state certificate

  • One year of experience working with population served in a nursing capacity

  • Current LPN/LVN licensure for state of residence

  • Other education and experience plus licensure as required by state such as CPR/FA, HIV/AIDS certification, BBP and ACT training

Work involves providing for the care, treatment, and general welfare of individuals. May supervise the work of others. Works under general supervision, with moderate latitude for the use of initiative and independent judgment. Performs basic and complex nursing tasks to fulfill the health needs of the most medically fragile individuals, including the collection of lab work and other health screenings. Based on the LVN experience they will assist the RN Supervisor with on-the-job Nursing instruction and training of other new LVN's. Assists the Infection Control nurse by implementing measures that prevent the spread of disease, assists the QA Nurse in developing and implementing quality assurance programs, and assists in the implementation of therapeutic nursing programs.Essential Job Functions: Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.1.Administers medications and treatments orally, rectally, internally, or by injection.2.Performs technical Nursing procedures according to facility and state regulations after demonstration for competency, i.e., tracheostomy care, gastrostomy/jejunostomy care, wound care, breathing treatments.3.Must be competent in use of all medical equipment, after competency is demonstrated.4.Documents and reports pertinent observations. Is able to recognize symptoms of illness and report accurately to RN and/or Primary Care Provider.5.Documents according to facility policy.6.Collects data for RN/Primary Care Provider.7.Serves as a resource/mentor for new LVN IIs.8.Collects specimens for laboratory examination and conducts appropriate screening tests (after competency is documented) when requested by lab personnel.9.Responds to emergency situations to ensure safety of individuals. Follows safety and Infection Control Procedures of facility.Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.Knowledge Skills Abilities: Knowledge of nursing procedures, techniques, principles and processes used in providing individualcare.
