Halifax Health Hospice is seeking a Licensed Practical Nurse (LPN).

Summary

The Licensed Practical Nurse will provide direct patient care to assigned patients under the direction and supervision of a Registered Nurse. Job responsibilities include carrying out selected procedures, administration of medications, and treatments.

Job Qualifications

  • Currently licensed or eligible for LPN licensure in State of Florida.

  • Graduation from an approved School of Nursing in a course of training for Licensed Practical Nurses.

  • Must have one year's experience in Hospice, Med-surg or other acute care area.

  • Valid driving license and good driving record are required for this position.

  • Professionalism in interpersonal communication skills with patients, patient families, colleagues, physicians and ancillary department personnel.

  • On-Call responsibilities required.

  • Certification from a 30 hour IV course preferred.

  • Additional certification provided through the facility for IV responsibilities after certification recieved.

Job Duties and Responsibilities

  • Provides compassionate care to the terminally ill and their families.

  • Perform and document complete assessments.

  • Reports any change to supervising RN.

  • Provides patient and family education.

  • Reinforces the implications of the diagnosis/prognosis and the nature of treatment based on information from primary nurse and/or physician.

  • Works in collaboration with an Interdisciplinary Team to develop a Plan of Care which reflect pt./family centered goals.

  • Uses critical thinking skills.

  • Functions as a representative of HOVF and utilizes effective communication methods and skills.

  • Participates in quality improvement activities.

  • Maintains professional and positive working relationships.

  • Promotes a holistic approach to the care of the terminally ill
