Qualifications for Licensed Practical Nurses (LPNs):

VIEW JOBS 5/15/2021 12:00:00 AM 2021-08-13T00:00 We are seeking a dynamic Nurse Practitioner to join us at Vanguard Medical Group for?our Verona, NJ location. The NP will work in a full-time position with the Primary Care at Home team, providing in-home clinical assessment and care to clinically complex patients within a 10-mile radius of the Verona office.? The responsibilities include: adult/geriatric assessment, medical history, physical exam, medication reconciliation, diagnosis and treatment, and health education. The right candidate will enjoy working independently and employ effective documentation using an EHR, and other clinical care services which may be required within the NP scope of practice.? The NP is expected to achieve VMG standards for quality, customer service, productivity and teamwork through participation in biweekly clinical meetings and other meetings as required. Position Responsibilities * Performs detailed health assessments (new patient, follow-up, sick visit) in patient homes or facilities; use of your own vehicle is required * Works collaboratively with physicians, care coordinators, social workers, family members, key care givers and any appropriate ancillary medical personnel * Reviews and interprets the medical and social history of patients * Performs appropriate screening procedures * Interprets laboratory data, recommends appropriate follow-up, reconciles medications * Is compliant with OSHA, State Board of Nursing * Participates in all mandatory compliance and other training programs, seeks guidance for compliance-related concerns and adheres to all applicable laws, regulations and Vanguard policies and procedures * May act as a preceptor for new NPs * Maintains positive control of all PHI information * Performs other duties as assigned Requirements * Knowledge, Skills and Abilities * Must be able to demonstrate strong competency with EHR and computer skills including use of e-mail and Microsoft Office programs * Must be able to demonstrate effective written communication skills including but not limited to writing reports and correspondence * Must be able to demonstrate effective verbal communication skills, including but not limited to speaking effectively to patients, providers, families and staff * Demonstrated ability to meet deadlines, and use time effectively and efficiently. * Demonstrated successful experience working in an environment of change. Effectively copes with change and uncertainty; responds and "shifts gears" comfortably * Demonstrated ability to establish and maintain effective relationships with patients, staff and Care Team Members, and gain their trust and respect * Uses logical methods to solve problems with effective solutions; is action oriented and sees challenges as opportunities * Baseline Clinical Knowledge * Proven ability to perform comprehensive, adult/geriatric physical exam * Demonstrated ability to identify, diagnose and document chronic disease in adults * Demonstrated ability to recognize conditions, characteristics, and behaviors that place complex or elderly patients at risk * Proven ability to pursue feedback, continuing education and professional development * Ability to effectively reconcile medications Pertinent Clinical Work Experience * 2+ years of clinical experience as a Nurse Practitioner in acute care, geriatric, adult or family practice setting Education * Preferred: Master of Science in Nursing * Required: Satisfactory completion of NP licensure Licensure/Certifications * Current, active and unrestricted New Jersey State registered nurse license and NP license * Board certification by either AANP or ANCC in Adult-Gerontology, Adult, or Family Practice * Current CPR BLS certification Travel Requirements * Travel to patient homes and facilities using your own vehicle * Must have valid NJ State driver license * Must meet company's automobile liability insurance standards and provide proof of coverage Vanguard Medical Group Verona NJ
