Qualified candidates must meet the following minimum qualifications:

  • Be a high school graduate or have completed a GED program

  • LPN licensure in good standing in the state in which you practice

  • One year experience as a professional nurse, or equivalent experience in trauma or acute care

  • Proficient in knowledge and use of computers and EMR software

  • Experience providing education and teaching preferred

  • Exceptional communication skills

  • Focus on quality patient care

Job Responsibilities include but are not limited to:

  • Provides skilled nursing care as outlined in the care plan

  • Provide education to clients and their caregivers regarding nursing care needs

  • Observe changes in physical and emotional condition of the client and report to supervisor any concerns

  • Coordinates with the interdisciplinary team to provide continuity of care

  • Submit timely documentation on all visits

POSITION SUMMARY

The Licensed Practical Nurse (LPN) provides nursing care and teaching to clients and families. She/he also functions as an assistant to the physician and registered professional nurse, and is supervised while in the field by the RN.

QUALIFICATIONS

High School Graduate or GEDGraduate of an approved school of professional NursingCurrent licensure in good standing with the state in which the LPN will be practicing

Minimum of one (1) year of experience as a professional nurse (hospital or nursing home)Computer knowledge

Ability to work independently with minimal supervision

Exceptional communication skills with focus on quality patient care and customer satisfaction

Is flexible, cooperative, team player in fulfilling all obligations

RESPONSIBILITIES

Provides skilled nursing care as outlined in the nursing care plan to include the following:

Nursing services, treatments and preventative procedures requiring substantial specialized skill as ordered by the physician

Initiating preventative and rehabilitative nursing procedures as appropriate for the client's care and safety

Observing signs and symptoms and reports reactions to treatments, including drugs and changes in the client's physical or emotional condition to the RN as necessary

Teaching the client and caregivers regarding the nursing care needs and other related problems of the client at home

JOB DUTIES

Submits all documentation accurately and in a timely fashion per policy

Participates in case conferences

Discusses with the supervisor problems concerning the clients and how they may be best handled

Discusses with the supervisor the need for involvement of other members of the health team such as the home health aide, physical therapist, speech therapist, occupational therapist, social worker, etc.Follows the nursing care plan for each assigned client

Coordinates care with the interdisciplinary team in order to provide continuity of care

Participates in staff development meetings

Continually strives to improve his/her nursing care by attending in-service education, through formal education, attendance at workshops, conferences, active participation in professional and related organizations and individual research and reading
