Garden Care Center - LICENSED PRACTICAL NURSE (LPN) Full Time/Part Time 11pm-7am PURPOSE OF YOUR JOB POSITION The primary purpose of your job is to provide direct nursing care to the residents and to supervise the day-to-day nursing activities performed by nursing assistants. Such supervision must be in Accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing Services or Nurse Supervisor to ensure that the highest degree of quality care is maintained at all times.

WORKING CONDITIONS •• Is involved with residents, personnel, visitors, government agencies/personnel, etc.; under all conditions and circumstances. •• Is subject to hostile and emotionally upset residents, family members, personnel, and visitors. •• Communicates with the medical staff, nursing personnel, and other department supervisors. •• Works beyond normal working hours, and in other positions temporarily, when necessary. •• Is subject to call-back during emergency conditions (e.g. severe weather, evacuation, post-disaster, etc.) •• Attends and participates in continuing education programs. •• May be subject to the handling of and exposure to hazardous chemicals. •• Maintains a liaison with the residents, their families, support departments, etc., to adequately plan for the residents' needs.

EDUCATION Must possess, as minimum, a graduate of an approved LPN program and a current LPN License. EXPERIENCE Experience is preferred, but not necessary. On the job training is provided.

SPECIFIC REQUIREMENTS •• Must possess a current, active license to practice as an LPN in this state. 4• Must be able to read, write, speak, and understand the English language. •• Must possess the ability to make independent decisions when circumstances warrant such action. •• Must possess the ability to deal with personnel, residents, family members, visitors, government agencies/personnel, and the'genera1 public. •• Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care. •• Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel. •• Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care. •• Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents. •• Must be able to relate information concerning a resident's condition. •• Must not pose a direct threat to the health and safety of residents and other individuals in the workplace. •• Must meet the general health requirements set forth by the policies of this facility which include a medical examination. •• May be necessary to assist in the evacuation of residents during emergency situations. ADMINISTRATIVE FUNCTIONS •• Direct day to day function of the nursing assistants in accordance with current rules, regu1ations, guidelines that govern the facility. •• Ensure that all nursing personnel assigned to you comply with the written policies and procedures established by the facility. •• Periodically review the department's policies, procedures manuals, job descriptions, etc. •• Cooperate with other resident services when coordinating nursing services to ensure that the resident's total regimen of care is maintained. •• Participate in the development, maintenance, and implementation of the facility's quality assurance program for the nursing service department. •• Periodically review the resident's written plan of care. Participate in the up ting of the resident's care plan as required. •• Assist in the planning of the nursing services portion of the resident's discharge plan as necessary. •• Admit, transfer, and discharge residents as required. •• Complete accident/incident reports as necessary. •• Perform administrative duties such as completing medical focus, reports evaluations, studies, charting, etc. as necessary. •• Assist in resident rehabilitation activities.

Provide preventative health services. •• Be familiar with and use as appropriate all items of personal protective equipment offered by facility, including, but not limited to, mask, gloves, and gown •• Participate in employee performance evaluations. Develop work assignments for the nursing assistants. •• Provide leadership to nursing personnel assigned to your unit/shift •• Make daily rounds of your unit to ensure that nursing service personnel are performing their work assignment in accordance with acceptable nursing standards. Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the unit and facility. •• Receive/give report upon start and end of your shift •• Report occupational exposures to blood/body fluids, infectious materials and hazardous chemicals in accordance with the facility's policies and procedures governing accidents and incidents. •• Ensure rooms are ready for new admissions.

Greet newly admitted residents upon admission. •• Participate in the orientation of new residents/families. •• Notify the resident's physician and family when an accident or incident has occurred. Notify the resident's physician and family when there is a change of condition. •• Maintain a clean environment including nursing station, medication room, pantry, and utility rooms. CHARTING •• Complete and file required record keeping forms/charts.

Receive telephone orders from physicians. •• Transcribe physician's orders to appropriate place in the medical record. •• Chart nursing progress notes in an informative and descriptive manner that reflects the care Fl"ovi5edto the resident, as wellQS.the resident's response to the care. •• Fill out complete occurrence reports. •• Report all discrepancies noted concerning physician's orders, diet change, charting error, etc., to the Nursing Supervisor. •• Fill out and complete transfer forms in accordance with established procedures. Perform routine charting duties as required and in accordance with established charting and documentation policies and procedures. •• Participate in the care planning process. DRUG ADMINISTRATION FUNCTION •• Prepare and administer medication as ordered by the physician. •• Verify the identity of the resident before administering medications/treatments. â• Ensure that prescribed medication is administered to the right resident. •• Ensure that an adequate supply of floor stock medications, supplies, and equipment is on hand to meet the nursing needs of the residents.

Report needs to the Nursing Supervisor. •• Order prescribed medications, supplies, and equipment as necessary, and in accordance with established policies. •• Ensure that the narcotic record is accurate for your shift Must coun.t narcotics with another licensed nurse at the beginning and the end of your shift. •• Notify the Nursing Supervisor of all drug and narcotic discrepancies noted. •• Review medication records for completeness, accuracy, and adherence of stop order policies. •• Notify the attending physician of automatic stop order prior to the last dosage being administered •• Assist in monitoring the inventory of medications, medical supplies, and equipment to ensure an adequate supply is on hand to meet the needs of the residents. TREATMENT ADMINISTRATION FUNCTION •• Report to the Nursing Supervisor any skin conditions noted on residents. •• Examine the resident and his/her chart to discriminate between normal and abnormal findings, in order to recognize when to refer the resident to a physician for evaluation, supervision, or direction. •• Perform duties such as completing wound care forms, reports, care plans, and evaluations. •• Complete weekly skin progress note documentation on all applicable residents. Ensure that the resident with pressure sores receive appropriate prophalaxis and treatment, such as turning and positioning schedules, pressure relieving devices, and nutritional supplements. •• Complete Braden Assessments on all new and readmissions, as well as on all residents on a quarterly basis. Implement preventative pressure relieving devices based on the score of the Braden assessment.
