Under general supervision of a Registered Nurse or Provider (NP, PA, MD, DO), the purpose of this position is to manage individualized, goal directed nursing care through use of the nursing process and the principles of primary nursing in accordance with departmental and company policies and procedures. This position will also assess patient health problems and needs, develop and implement nursing care plans, and maintain medical records and administer nursing care to ill, injured, convalescent, or disabled patients. While this role is intended to serve at a particular APM location, it is within APM?s right to assign staff to other APM locations when there are staffing shortages. Staff shall be reimbursed for their mileage for such assignments pursuant to APM policy

Essential Functions:

  • Provide skilled nursing care as directed. Consult and coordinate with health care team members to assess, plan, implement and evaluate patient care plans.

  • Use of specialized clinical and surgical equipment.

  • Observes and documents patients? medical and educational needs to assist in patient assessment and care.

  • Consistently follows the Nursing Plan of Care for the patient.

  • Accurately monitor, record and report on patients? medical information and vital signs.

  • Instruct individuals, families and other groups on topics such as health education, disease prevention, and develop health improvement programs.

  • Assists in pre/post operation by preparing patient for procedure; correctly identifying patient, procedure to be performed and procedure site. Updates and confirms medication and allergy profiles, completes appropriate clinical lab work (glucose reading, INR, pregnancy test, toxicology screen as indicated), completes and documents vital signs, pain status, HPI. Reinforces pre and post op teaching.

  • Obtains and documents modified Aldrete and discharge scoring systems. Assists in discharge of patient with appropriate paperwork.

  • Perform chart review on daily basis to ensure completeness and accuracy of patient documentation.

  • Assists in keeping ASC orderly and follows appropriate cleaning protocols.

  • Act as scribe for provider recording patient information as dictated, when requested.

  • Explain treatment procedures, medications, diets and physicians' instructions to patients.

  • Participates and maintains safety and dignity of patient at all times.

  • Effectively communicates with all providers and staff members.

  • Driving may be required when working in clinic roles.

  • All other duties as assigned.

Hours are 5:00 AM - 1:00 PM with rotating Saturdays from 5:00 AM - 10:30 AM Madison East CTC, a member of Acadia Healthcare's Comprehensive Treatment Centers, is a part of the leader in medication assisted treatment for individuals struggling with an opioid addiction. With full-circle care that includes a blend of therapies and the use of safe and effective medications, we are prepared to treat the entire problem, not just a piece of it. Position Characteristics The Licensed Practical Nurse (LPN) provides basic medical care to patients in an outpatient addiction recovery environment; maintains standards of professional nursing practice in accordance with clinic policy and procedures; provide for the safe and efficient dispensing of doses of medication to patients consistent with the organization's procedure and as ordered by the Medical Director or Physician; responsible for thorough, accurate reporting and documentation of the patient's symptoms, responses and progress. Responsibilities * Works under the supervision of the Medical Director and the Clinic Director and maintains practical nursing professional standards of care; * Prepares and administers medication as ordered by the Medical Director or Program Physician accurately and in a timely manner, maintains accurate accounting of all medication received (from supplier) and dispensed (to the person(s) receiving services); * Chart medication administration in nurse's notes accurately and in a timely manner, documents nursing care provided, maintain appropriate records related to patient's progress and activities accurately and in a timely manner; * Responsible for the safekeeping of all medication stock and the records required by the DEA, FDA, and ADP; * Dedicates a minimum of 90 percent of regularly scheduled work hours to activities directly related to the administration of medication; the remaining 10 percent of regularly scheduled work hours will be dedicated to other activities as assigned by the Medical Director or Clinic Director; * Responsible for urine collection and processing, reading test results as ordered by Program Physicians and the completion of all forms involved in these procedures; and, * Perform other duties as assigned. Job Requirements * Graduation from an accredited school of nursing * Prefer 1 year nursing experience in behavioral health or addiction recovery abuse field * Training or experience treating patients in an outpatient treatment program * There must be no history of alcohol or other drug misuse for a period of three years before employment Licenses/Certifications: * Current Licensed Practical Nurse (LPN) license in the state of Wisconsin.
