Examples of Duties Performed:

  • Responsible for the safe and efficient management of a long-term care patient area and/or nursing department shift.

  • Plans and directs the activities of assigned staff and oversees the execution of quality patient care.

  • Complies with current federal, state, and local regulations governing nursing care in the execution of assigned duties.

  • Administers and records medications following physician orders and established center policy

  • Takes and records patient's vital signs and collects laboratory specimens as directed.

  • Provides emotional support to patients and their families.

Position Overview Responsible for facilitating all patient discharges and transfers from the Inpatient Unit. The Discharge Planner functions as coordinator of discharge plans, collaborating with the patient and caregiver, the patient's care team, outside facilities and DME vendors, to ensure safe and timely patient discharges and transfers. Key Position Qualifications * Active Georgia Nursing License * 2-3 years RN/LPN experience in a clinical setting * Specialized experience or certifications in related field; oncology preferred * Superb customer service, telephone, and communication skills * Ability to show compassion to a diversified group of oncology patients * Excellent organizational and prioritization skills * Ability to work independently with minimal supervision * Must possess the ability to utilize clinical reasoning while ensuring that coordination of services are done in a timely manner * Demonstrates sound knowledge and actions in the care and decision making for the oncology patient population and seeks guidance appropriately * Possesses a basic knowledge of personal computer and software for word processing and/or good keyboard skills preferred, or the ability to enter and retrieve data from relevant computer systems The Best Place to Work We call ourselves Stakeholders because we all have a stake in the care and success of every patient.
