About the Job

Assumes responsibility for providing skilled, creative, high quality nursing care to patients under the direction and supervision of a registered nurse. Performs a variety of direct and indirect patient care duties and responsibilities in collaboration with other nursing staff. Is accountable for safely managing the administration of medication and monitoring effectiveness.

Qualifications:

Valid and current Massachusetts licensure as a Licensed Practical Nurse, current certification in CPR. Experience with addiction treatment preferred.

Must demonstrate ability to relate to a substance abuse treatment population in a sensitive manner, an interest and willingness to work in a multicultural setting and must share a customer service orientation. Identifies and resolves problems in a timely manner; Uses reason even when dealing with emotional topics. Manages difficult or emotional client situations; Responds promptly to client needs and maintains confidentiality.

VIEW JOBS 4/24/2019 12:00:00 AM 2019-07-23T00:00 GENERAL SUMMARY/ OVERVIEW STATEMENT: The LPN is a licensed nurse who participates in the plan of care to provide competent, compassionate and individualized nursing care to patients and their families – as developed by a registered nurse, nurse practitioner, or physician. Their role is crucial in facilitating clinic flow during clinic sessions by taking vital signs, rooming patients, administering vaccines, performing phlebotomy, assisting with procedures, providing wound care, PICC care and other skilled tasks. The LPN verifies and updates patient information in EPIC, supports prescription refills and performs other duties to support patient care and the clinical teams. As a member of the care team, the LPN will foster therapeutic long-term relationships with their patients and contribute their insights to the care team to improve the patients plan of care. PRINCIPAL DUTIES AND RESPONSIBILITIES: Patient Care * <li> Performs tests, procedures, and treatments within the LPN scope of practice. * Obtains vitals signs and patient fall screens prior to patient appointments and documents these findings in a timely manner. * Administers vaccinations and updates immunization records with documentation from outside facilities. * Administers PO/IM/IV medications per order and documents the administration in a timely manner in the patient's chart. * Includes placement of peripheral IV as needed * Assists with procedures or specimen collection by providers in clinic. C. Obtains vital signs such as orthostatic or follow up vital signs as requested by the care team * Provides PICC line dressing changes, Cathflo and PICC removal per order * Respects and supports patient and family's cultural beliefs and values, individual needs, health goals and treatment preferences. Administrative Duties * <li> Participates in the Outpatient Parental Antimicrobial Treatment program * Monitors their own small panel of patients through out the duration of their course of antibiotic therapy at home or in a skilled nursing facility. * Tracks and transcribes outside labs weekly in EPIC and alerts care team to abnormal values and lab trends. * Sending orders to community care team to convey changes in care plan as ordered by the treating ID physician. * Monitors Prescription refill line daily * Transcribes refill requests electronically in EPIC and forwards to the care team * Performs daily quality control measures * Monitoring code cart, vaccine refrigerator temperatures and controlled substance count. Collaboration * Works with Medical Assistants and team to facilitate clinic flow * Works collaboratively with RN to create and implement plans of care during clinic visits. * Provides insights on patient to care team to assist in care planning. * Assists with primary care and specialty sessions (eg ID Neurology ) as needed. Other * <li> Assumes other responsibilities & duties within the realm of the licensed nurse * Participates in quality assurance activities of the clinic * Works as team member with other members of the nursing staff Professional development * Maintains and updates skills based on current nursing practice. * Adheres to departmental and hospital wide requirements for required training. * Identifies specific learning needs and goals and collaborates with leadership in developing a plan to meet them. WORKING CONDITIONS: Patient care in an outpatient, academic, ambulatory specialty and primary care setting. SKILLS/ABILITIES/COMPETENCIES REQUIRED: Proficiency in wound care, venipuncture, and injection administration.
