Position Overview

Hiring Immediately As a Licensed Practical Nurse (LPN), you will implement and coordinate the delivery of care in collaboration with physicians and care associates. You will be responsible for the high level of care our residents are accustomed to, including making frequent rounds, ensuring continuity of care, documenting all pertinent care information, and observing our residents' overall condition and behavior. Above all, you will exhibit a passion for serving seniors and contributing to the positive environment our residents are proud to call home.

  • Utilize the nursing process (assessment, planning, implementation, and evaluation) for the best care of our residents

  • Smile and Show Love, especially in trying times

  • Provide health and wellness checks to our residents on a regular basis

  • Assist the Resident Services Director in the development and update of individual service plans for each resident

  • Maintain ongoing communication with residents, their families, and their physicians regarding their needs

  • State LPN in good standing

The Licensed Vocational/Practical Nurse provides hemodialysis therapies to patients with chronic renal disease.</p><p>RESPONSIBILITIES:</p><ul><li><p>May contribute to and implement the plan of care as directed by the Registered Nurse and document as needed.</p></li><li><p>Monitor the outcomes of dialysis treatment on an ongoing basis and communicate results to the Charge Nurse or Administrator as appropriate.</p></li><li><p>Initiate and terminate dialysis treatments, within scope of practice and as allowed by state regulations.</p></li><li><p>Mix acid and bicarbonate according to center procedure when needed.</p></li><li><p>Participate in infection control monitoring, implementation, and recording as requested.</p></li></ul><p>REQUIREMENTS:</p><ul><li><p>Current nursing license in the applicable state.</p></li><li><p>Confirmation of ability to distinguish all primary colors.</p></li><li><p>Previous dialysis nursing experience preferred but not required.</p></li><li><p>Minimum 6 months experience in nursing preferred.</p></li></ul><p>U.S. Renal Care is an Equal Opportunity Employer/Disabled/Veteran.</p><p>Qualifications/Requirements:</p><p>To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Requirements include:</p><ul><li><p>Current LVN/LPN license in applicable state. License must be maintained as current and in good standing.</p></li><li><p>CPR certification required within 90 days of hire.</p></li><li><p>Confirmation of ability to distinguish all primary colors.</p></li><li><p>Previous dialysis experience preferred.</p></li><li><p>Successful completion of USRC training program approved by the Medical Director, including demonstrated competency, within 8 weeks of hire.</p></li><li><p>Demonstrated working knowledge of the English language and ability to communicate verbally and in writing.</p></li><li><p>Must be able to organize time and tasks well.</p></li><li><p>Must have basic computer skills; proficiency in all USRC clinical applications required within 90 days of hire.</p></li><li><p>Must meet any practice requirement(s) for the applicable state.</p></li></ul><p>Successful Competency Factors:</p><p>Use of the following skills: problem solving, analytical ability, effective communication, confidentiality. Interpersonal skills necessary to work with patients, staff, and all other individuals as required.
