Responsibilities for Licensed Practical Nurse (LPN):

Qualifications for Licensed Practical Nurse (LPN):

  • Current LPN license in good standing

S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Advanced Practice Nurse - PPV provides hospice and palliative care for patients facing chronic, complex and/or life-threatening conditions at various locations within the flow of patient care. S/he provides assessments, educational, and evaluative services to meet the needs of hospice and palliative care patients and their families. S/he serves as clinical and educational role model and mentor; provides care and support in collaboration with the primary care team; and ensures compliance with practice standards and codes. PRINCIPAL ACCOUNTABILITIES * Promotes palliative care and hospice philosophy. * Communicates identified needs and potential solutions to supervisor. * Performs routine and emergency assessments. * Performs bedside palliative care patient/family evaluation. * Orders, performs, and interprets laboratory and radiology tests within scope of professional practice. * Prescribes medications including controlled substances to the extent delegated and licensed. * Orders treatments and durable medical equipment as indicated. * Performs other therapeutic or corrective measures as indicated. * Consults with palliative care physician or designees as needed, informs primary physician of services provided and collaborates with other physicians as needed. * Assists in all facets of care coordination for palliative care referrals. * Prepares and maintains timely, accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided. * Complies with applicable laws and regulations with respect to Collaborative Agreements. * Initiates reimbursement for services rendered. * Provides training and continuing education for staff. * Assists in development of clinical practice guidelines/standards in support of quality palliative care. * Advocates for improvements in palliative care through participation in professional, private and governmental groups. * Defines goals for professional growth and participates actively in professional activities and organizations. * Engages in active and frequent self-care activities for personal and professional growth and longevity. * Supports program and corporate policies, goals, and objectives. * Promotes a positive working relationship between agency and corporate personnel. * Attends and participates in staff meetings, educational programs and community events, as requested. * Adheres to the practice of confidentiality regarding patients, families, staff and the Company. * Assists patients and families in identification of goals of treatment and ongoing plans of care. Communicates these goals among Interdisciplinary Team (IDT). * Applies the rules and regulations of state and federal regulatory agencies and other certified agencies. * Helps the patient and family maintain their maximum level of comfort and coping. * Collaborates and communicates with Medical Director, Attending Physician, Hospital Staff and IDT. * Collaborates and communicates with IDT needs of patient for clinical and psycho-social interventions. * Assists in identifying the need for intervention of other IDT members. * Effectively communicates patient and family needs to IDT. * Completes and submits required paper work within 24 hours of visit completion. * Educates patient and caregiver regarding: * Care of patient. * Disease process. * Goal Setting. * Symptom control. * Treatment Options. * Completes a minimum of 37 visits/month in an established market with the availability to complete same-day consult request. * Ability to triage consults by patient acuity. * Provides a minimum of 2 educational programs/week to promote and educate a variety of audiences about palliative care services. * Works closely with the Director of Strategic Partnership to meet customer(s)' needs. * Participates in facility and partnership meetings to promote collaboration with all aspects of patient care. * Supervisory Responsibilities: * Supervision of nursing trainees, including graduate and advance practice nurses in field placements. * Participation in supervision of other trainees, including medical students, residents, fellows, and social work and psychology trainees. * Educates new Advanced Practices Nurse - PPV orienteers per request. * Performs other duties as assigned. KNOWLEDGE AND SKILLS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education and/or Experience * Licensed to practice in state of work. * Minimum of 5 years nursing experience; clinical nursing care in oncology, hospice, geriatrics, intensive care preferred. * Master's Degree in Nursing. * Certified, or eligible, as a Clinical Nurse Specialist or Nurse Practitioner. State Specifics: * All States except Iowa: Current collaborative physician agreement in place. Mathematical Skills Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage. Language Skills Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, colleagues, investors, and external parties. Strong written and verbal communications. Reasoning Ability Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, or schedule form. Computer Skills Proficiency in Microsoft Office Suite. Other Qualifications Strong organizational and time management skills. Must be able to work a flexible schedule to include nights and weekends. Must be able to travel. Certifications, Licenses and Registrations CNS or Nurse Practitioner License in state of employment. Must have a valid driver's license and auto liability insurance. Board Certification in Hospice and Palliative Care is preferred, but not required. Other Skills and Abilities Ability to understand, read, write, and speak English.
