This position is located at the Department of Mental Health, Waccamaw Center for Mental Health, 501 Nelson Blvd, Kingstree, SC 29556.

As the LPN II, under general supervision, you are responsible for planning, implementing and providing direct nursing care to all patients of the Kingstree Office for Waccamaw Center for Mental Health. In collaboration with the treatment team, evaluates the effectiveness of care/interventions and discharge planning.

Responsibilities of the LPN II include but are not limited to:

  • Provides psychiatric nursing services to all patients with chronic and persistent mental illnesses, and children/adolescents with severe emotional disturbances. Services may include making observations, administering medications (oral and injections), providing counseling and education to patients and families, and utilizing other appropriate resources as needed. Will perform urine drug screens and other necessary tasks as requested by immediate supervisor and Clinic Director.

  • Maintains documentation of charts and other records in accordance with State, Federal, Local, SCDMH, WCMH, Quality Assurance, Corporate Compliance and

CARF Standards.

  • Completes and submits clinical service notes within a 24-hour period in accordance Local Collaborative Documentation Standards.

  • Works directly with staff psychiatrists/ physicians to coordinate medication needs and address medication concerns. Participates in treatment team meeting to gain assistance in the formulation and implementation of these plans. Promotes and helps maintain the health of individuals and families through teaching, counseling, and rehabilitative measures. Coordinates needed community supports and works closely with Community Support Staff members.

  • Will assist with duties to include, but not limited to; assisting with stocking medications, helping to triage patients when needed, helping provide coverage for patients when other nurses are out of the clinic, follow up calls for physicians/ nurses and completing forms for labs when needed, assisting with referrals from physicians as needed. Wil also particiate in staffing's and meetings. Assists with nursing/medical consultation to team members. Locate, Coordinate and monitor needed medical, social, educational, self-help, supportive entitlement and other services; consult with other professionals regarding client care, including family and support systems.

Minimum and Additional Requirements

State Minimum Requirements:

A bachelor's degree in nursing and 2 years of work experience OR a 3-year diploma and 2 years of work experience. Must have current South Carolina State License from SC Board of Nursing.

Agency (Facility) Minimum Requirements:

A bachelor's degree in nursing and 2 years of work experience OR a 3-year diploma and 2 years of work experience. Must have current South Carolina State License from SC Board of Nursing.

Must have graduated from an accredited school of practical nursing and related experience.

Must be able to meet DMH credentialing requirements.

Preferred Qualifications

Bilingual abilities in English and Spanish (or another language) are a plus.

Additional Comments

Thorough knowledge of the services provided in a comprehensive mental health program serving children and families and adults. Extensive skills in coordinating services with other health and community organizations. Ability to establish and maintain satisfactory working relationships with employees. Ability to establish and maintain working relationships with patients and families. Employee with serve patients of Kingstree Office.
