Licensed Practical Nurse, Gvmp Medical Practices

Responsibilities

The Licensed Practical Nurse serves the patients at Grand View Practices collaborating with patients and their primary care providers to ensure the delivery of quality, efficient, and cost-effective healthcare services. The Licensed Practical Nurse is accountable for assessing, planning, delivering and evaluating care for patients served and collaborates with other members of the health care team to achieve optimal patient outcomes. This position will work as needed in several Grand View Medical practice locations. Practices are located in Sellersville, Hatfield, Harleysville, Chalfont, and Quakertown

Essential Functions:

• Ensures physicians have information necessary to provide exceptional care.

• Greets, escorts and interviews patients and prepares exam rooms for physician.

• Reviews daily incoming office, enters into the EHR for physician reviews and follows up with office physician as necessary

• Utilization of computer to gather information, review patient data and schedule appointments.

• Review of medications and prescription requests, including notification of need for appointment.

• Coordinates appropriate laboratory and diagnostic testing

Qualifications

Education and Experience:Vocational diploma Lisenced Practical Nurse Training And Over 6 months to 1 year Acute care setting required
