The Licensed Practical Nurse in the Family Health Center for this position must have a high level of experience in the care of the pediatric patient from birth to age 18. This position is an integral part of the health care team. The LPN in pediatrics must have knowledge and and skills of vaccine administration and must be able to assume responsibility of the Vaccine For Children program. The LPN will provide nursing support to the professional team by providing direct patient care in terms of managing a high volume of patients coming into the Family Health Center for a variety of appointment types. The LPN will interview and room patients, ensuring all regulatory requirements are maintained. The LPN will provide follow up for various clinical sessions, and will maintain care in the appropriate documentation format, meeting all Joint Commission and patient safety requirements. Works closely with physicians to ensure plan of care is complete and the patient understands the plan. Provides instruction and education to patients and families. Coordinates medication refills, phone calls, and appointments. Must provide care in an organized and patient centered manner. The LPN will maintain all required nursing competencies on a yearly basis. Able to act in accordance with standards in case of patient emergency. Must be able to communicate in a positive manner with staff, patients and physicians. Must have a basic understanding of insurance plans and implications on the path for care. Must be able to work closely and collaboratively with physicians, nurse practitioner, and team members.
Inventory Control of the Vaccine for Children Program on a daily basis, including daily data entry of lot numbers and control of inventory.
Maintains lead quality control program.
Administration of Medications
Vaccine administration to infants and children
Rooming of patients including vital signs, nursing intake, updating health summary
Documents in the medical record in accordance to standards
Manages a large volume of phone calls, coordinates prescription refills
Filing, management of the integrity of the medical record
Coordinates care between primary and specialty care and assists the patient with navigation of the health care system
Other duties as assigned including assistance with venipuncture of adults, EKGs, and working other clinical areas of the Family Health Center if needed.
Scheduling of procedures, tests
Performs other duties as needed such as completing referrals and coordinating prior authorizations
This job may require being available after hours should there be a question of vaccine integrity during the hours the Family Health Center is closed.
This LPN will be responsible for the integrity of the Vaccine For Children program in accordance with all rules, regulations and standards of the New Jersey Department of Health and the CDC.
Current and valid New Jersey Practical Nurse license required
Active American Heart Association (AHA) BLS/CPR certification required
At least two years of outpatient office setting pediatric nursing experience required
Up to date education credentials in the Vaccine for Children program requirements preferred
Two years experience entering data for the VFC program preferred
Fluent in the Spanish language preferred. Qualified interpreter status preferred.
