Responsibilities

The Licensed Practical Nurse in the Family Health Center for this position must have a high level of experience in the care of the pediatric patient from birth to age 18. This position is an integral part of the health care team. The LPN in pediatrics must have knowledge and and skills of vaccine administration and must be able to assume responsibility of the Vaccine For Children program. The LPN will provide nursing support to the professional team by providing direct patient care in terms of managing a high volume of patients coming into the Family Health Center for a variety of appointment types. The LPN will interview and room patients, ensuring all regulatory requirements are maintained. The LPN will provide follow up for various clinical sessions, and will maintain care in the appropriate documentation format, meeting all Joint Commission and patient safety requirements. Works closely with physicians to ensure plan of care is complete and the patient understands the plan. Provides instruction and education to patients and families. Coordinates medication refills, phone calls, and appointments. Must provide care in an organized and patient centered manner. The LPN will maintain all required nursing competencies on a yearly basis. Able to act in accordance with standards in case of patient emergency. Must be able to communicate in a positive manner with staff, patients and physicians. Must have a basic understanding of insurance plans and implications on the path for care. Must be able to work closely and collaboratively with physicians, nurse practitioner, and team members.

  • Inventory Control of the Vaccine for Children Program on a daily basis, including daily data entry of lot numbers and control of inventory.

  • Maintains lead quality control program.

  • Administration of Medications

  • Vaccine administration to infants and children

  • Scheduling appointments

  • Rooming of patients including vital signs, nursing intake, updating health summary

  • Documents in the medical record in accordance to standards

  • Manages a large volume of phone calls, coordinates prescription refills

  • Filing, management of the integrity of the medical record

  • Coordinates care between primary and specialty care and assists the patient with navigation of the health care system

  • Other duties as assigned including assistance with venipuncture of adults, EKGs, and working other clinical areas of the Family Health Center if needed.

  • Scheduling of procedures, tests

  • Performs other duties as needed such as completing referrals and coordinating prior authorizations

  • This job may require being available after hours should there be a question of vaccine integrity during the hours the Family Health Center is closed.

  • This LPN will be responsible for the integrity of the Vaccine For Children program in accordance with all rules, regulations and standards of the New Jersey Department of Health and the CDC.

Qualifications

  • Current and valid New Jersey Practical Nurse license required

  • Active American Heart Association (AHA) BLS/CPR certification required

  • At least two years of outpatient office setting pediatric nursing experience required

  • Up to date education credentials in the Vaccine for Children program requirements preferred

  • Two years experience entering data for the VFC program preferred

  • Fluent in the Spanish language preferred. Qualified interpreter status preferred.

Responsibilities A day in the life of a Registered Nurse at Hackensack Meridian includes: * Delivery of nursing practices according to the most up-to-date evidence-based research, nursing policies, and standards of care, to provide and ensure safe and excellent delivery of individualized patient care. * Ongoing communication with patients, nursing colleagues and other members of the health care team regarding patient's status, rendered care and treatment plans. * Medication administration utilizing the newest advancements in bar-scanning and documentation technologies designed to ensure the safest delivery of medications to our patients. * Ongoing Point-of-Care electronic documentation of each individual patient's assessments, plan-of-care, interventions and evaluation of rendered care. * Patient and family centered education aimed at optimizing health, wellness and disease management. * Multi-disciplinary discharging of patients to ensure safe transitioning and continuum of care * Rapid nursing assessment * Patient interviews and comprehensive nursing initial assessments * Patient and family education * Care coordination and follow up, including making appointments, follow up on tests and procedures, assisting patients in following instructions * Phone management * Prescription management * Development and update of the health summary * Working closely with interdisciplinary team members, both on site and throughout the hospital system. * Continual dialogue physicians and physician offices Qualifications * Current and valid New Jersey Registered Nurse license is required * BLS/CPR certification by American Heart Association is required * One or more years of Registered Nurse experience required * At least one or more years working in an outpatient setting, preferably in an outpatient primary care or specialty office setting as a registered nurse. * Able to confidently administer vaccines * Ability to demonstrate rapid assessment skills for a wide range of adult conditions and provide care coordination to a variety of patient populations is highly desired. * Ability to respond to a high volume of patients in an outpatient setting. * The ability to learn quickly and adapt to changing patient needs, a strong sense of accountability for improving the lives of our patients and caregivers, an exceptional focus on teamwork and flexibility, dedication to ongoing education and the ability and passion to deliver the highest quality of patient centered education are all required.
