Our Family Practice River Road office is hiring two full time LPNs to join their team! The hours are Monday-Friday 7:30am-4:00pm. A late-night may be required based on the provider's schedule during the week.

Responsibilities:

  • Document verbal conversations with patients when appropriate.

  • Document information into patient record in an accurate, legible and timely manner.

  • Maintain timely patient flow.

  • Prepare patient for visit as appropriate, based on office/provider requirements.

  • Obtain specimens (i.e., urine, blood, etc.) from patients as directed by the provider.

  • Accurate and timely order entry into the lab system.

  • Perform clinical office procedures as directed by the provider (i.e. PFT, EKG, nebulizer, medication administration and immunizations)

  • Administer and record vaccines.

  • Process patient prescriptions.

  • Discuss test results with patients per provider's instructions.

  • Previous LPN experience in a medical practice or prior experience as a Medical Assistant or Patient Care Tech.

Candidates who are not currently on the eligible list may apply for this position and if appointed, will receive a temporary appointment. To qualify, candidates must have one of the following:</p><ul><li>A license and current registration to practice as a Licensed practical Nurse in New York State;</li></ul><p>OR</p><ul><li>A limited permit to practice as a Practical Nurse in New York State.</li></ul><p>OR</p><ul><li>An application on file for a limited permit to practice as a Practical Nurse in New York State. (You must be approved immediately following graduation from an approved school of practicing in New York State to qualify under this option.</li></ul><p>Note 1: If you are appointed with a limited permit or have an application on file for a limited permit, you MUST obtain your license within one year of the date of your limited permit was issued or steps will be taken to remove you from the position.</p><p>Note 2: Candidates appointed temporary must receive a score that is reachable on the resulting eligible list before they can be permanently appointed. If you fail to maintain your license to operate a motor vehicle in New York State, appropriate measures may be pursued to remove you from this position.</p><p>Duties Description • Monitor the conditions of the Individuals in their assigned worksite in order to detect indications of changes in condition or responses to medication or therapy.</p><ul><li><p>Identify symptoms of illnesses and changes in conditions manifested by individuals, and report such symptoms or changes to a physician or registered nurse in order to provide information useful in diagnosis and treatment planning.</p></li><li><p>Take temperatures, pulse rates, blood pressures and respiration rates using thermometers, manometers, stethoscopes;</p></li><li><p>Administer all injectable, j-tube and g-tube medications to assigned individuals.</p></li><li><p>Implement and monitor the g-tube and j-tube feedings, including changing g-tubes as necessary.</p></li><li><p>Implement catheterization orders, trach care, deep oral and pharyngeal suctioning.</p></li><li><p>Assist individuals in collecting urine, stool and other specimens in order to obtain such samples for analysis.</p></li><li><p>Prepare and maintain charts and other records and treatments administered in order to document such conditions and activities.</p></li><li><p>Perform inventory of current medical supplies and medications to determine the need for ordering new supplies or medications.</p></li><li><p>Perform &quot;Direct Care&quot; duties as assigned.</p></li></ul><p>Additional Comments • Please note, the posted Hiring Salary includes the sum of the statutory hiring rate for ISU Grade 10 ($39,653), along with any special salary treatments in effect for this position in this location, such as a Geographic Differential ($4,000) and increased Hiring Rate ($5,148). The posted maximum salary (Job Rate) includes the previously mentioned Geographic Differential.</p><ul><li><p>As a participating agency in the Health Care and Mental Hygiene Worker Bonus Program (HWB), direct care employees may be eligible to earn up to $3000 in bonuses for qualifying service if they meet specific requirements including continuous service during the appropriate vesting periods.
