Open Sky Community Services is a human service agency that provides counseling services and treatment options to people diagnosed with a mental illness or developmental disability. The Adult Learning Center is a Day Habilitation program currently serving 45 adults between the ages of 23-85 with a diagnosis of intellectual disabilities and some physical challenges.
Most participants require supportive structures and staff assistance to maximize their involvement in activities. Services and supports provided to participants include, but are not limited to: occupational therapy, speech therapy, physical therapy, nursing oversight, functional skill development, communication, socialization, community membership, and self-advocacy
The Licensed Practical Nurse works in close collaboration with the Registered Nurse to ensure the medical needs of the person supported is met. This position involves acting as a liaison between the program and other stakeholders including residential program, family members, and other medical providers.
The LPN can expect to provide 1:1 support to the person served while at the program and while out in the community. This 35-hour position is first shift Monday-Friday.

Develop and implement the Day Hab Service Plan with other members of the interdisciplinary team
Teach skills and create opportunities for practice, including data collection
Attend to all personal hygiene routines/needs
Current Massachusetts Registration. At least 1 year experience in a health care setting working with persons who have developmental disabilities. SOLVE certified (within 6 months of hire).
