Job Requirements

Essential Duties and Responsibilities

  • Ensures orientation of new patients immediately upon admission.

  • Obtains history and presenting information as delineated in program policy.

  • Collaboratively develops and updates an individualized treatment plan with the individual that includes targeted discharge date and plan.

  • Defines all relevant community sources affecting care and treatment of the consumer (e.g. attorney, family, case manager, etc.) and develops and follows contact plans and conjoint therapy plans.

  • Updates referring sources on patient progress, if/as approved by the individual in services.

  • Utilizes agency endorsed patient-centered therapeutic interventions and rapport-building practices, including motivational interviewing techniques, to non-confrontationally enhance patient insight regarding diagnostic and usage patterns.

  • Documents treatment episodes, contacts, treatment plans and sessions according to periodicity and content requirements.

  • Maintains appointed continuous caseload of actively engaged individuals.

  • Responds to crisis as needed.

  • Coordinates comprehensive discharge plan to ensure seamless community reintegration.

  • Performs all other duties as required or assigned.

Knowledge, Skills and Abilities

  • Ability to inspire client confidence in the FRN Staff and the FRN Model of Treatment.

  • Ability to consistently demonstrate through conduct and communications the stability required to effectively assist patients and their family members through the FRN Model of Treatment.

  • Excellent verbal and written communication skills required.

  • Excellent organizational skills with the ability to prioritize workload and meet deadlines.

  • Must be able to work in a fast-paced environment with demonstrated ability to juggle multiple competing tasks and demands.

  • Exceptional analytical and problem solving skills.

  • Proven ability to handle multiple projects under pressure.

  • Must have high level of interpersonal skills. Position continually requires demonstrated poise, professionalism, tact and diplomacy.

  • Must have strong knowledge of a variety of computer software applications in word processing, spreadsheets, and presentations (MS Word, Excel, and PowerPoint).

  • Understanding of diverse cultures and gender specific issues and the ability to incorporate needs of gender and culturally diverse groups into the organization.

  • Understands need for and maintains appropriate confidentiality at all times when interacting with patients, residents, families, visitors, referral sources and all other contacts.

  • Exhibits excellent customer relation skills as evidenced by supportive and constructive communication with all contacts including coworkers, patients, residents, visitors, families and referral sources.

  • Knowledge of case management for dual diagnosis patients.

Education and Experience

  • Master's degree in counseling, psychology, social work, or related field required.

  • Licensure in a mental health related field is also required.

  • Minimum 2 years of experience in mental health or substance abuse field preferred.

Location BLH Bronson Lakeview Hospital Title BLH BHU on call Medical Social Worker Hospital Medical Social Worker: Responsible for the delivery of social work services to all medical areas of the hospital to a variety of patient populations. Provides direct intervention with patients and families in such areas as mental health, suicide, domestic violence, sexual assault, child/adult abuse/neglect, death/grief, crisis intervention, advocacy, and substance abuse. In conjunction with assessment provides intervention for families and patients for discharge planning, etc. Educates and counsels patients/families on available community resources and facilities. Collaborates with other members of the patient care team including external agencies to assess each situation to determine the best course of action. Coordinates and manages systems and activities toward the end that referred patients are discharged in a timely manner to psychiatric facilities, substance abuse treatment, etc. Employees providing direct patient care must demonstrate competencies specific to the population served. Practice Medical Social Worker: Responsible for the delivery of social work services in the ambulatory medical practice setting to a variety of patients including pediatric, adolescent, adult, and geriatric populations. Coordinates care to ensure patient/family access to an array of community services. Administers behavioral health screenings and conducts and arranges for more detailed assessments when indicated. Delivers brief interventions to support necessary lifestyle and behavioral changes. Responds to crisis situations in the ambulatory setting. Identifies high-risk psychosocial needs of patients/families. Collaborates with patients, families, and the interdisciplinary team to ensure understanding of the impact the disease or illness has on the patient's mental and emotional health. Offers a range of brief, focused prevention, treatment and recovery services for patients with mild to moderate risk factors. Works effectively as part of the interdisciplinary health care teams. Provides culturally competent clinical services which are relevant to patients/families. Emphasis of duties may vary depending upon the area of assignment. Hospital Medical Social Worker: Master's degree in Social Work and 1 year recent medical or mental health experience required Additional education/experience may be required depending on specific department Current Social Work license in the state of Michigan Current driver's license in the state of Michigan and proof of insurance required for BAH * Ability to move about the hospital to meet with patients and/or families in the patient care units * Basic Word Processing Skills * Demonstrates excellent human relation skills including verbal and written communication, problem solving, advocacy, assertiveness and empathy skills * Has a customer service orientation * Ability to communicate clearly and effectively, both verbally and in writing, as a demonstration of the function of effective process and relationships with the patient and members of the interdisciplinary team * Demonstrates effective interviewing, problem-solving and critical thinking in providing services to patients * Regularly utilizes effective negotiation and conflict resolution skills as needed. Work which produces high levels of mental/visual fatigue, e.g., interactive and repetitive or small detailed work requiring alertness and concentration for sustained periods of time, the operation of and full attention to a personal computer or CRT between 40 and 70 percent of the time. Involves regularly lifting of bulky or moderately heavy weights (i.e., up to 50 pounds), and occasionally assisting with heavier tasks or expending the equivalent effort in pushing, pulling, or otherwise handling material, equipment, and other objects. Proof of valid auto insurance on that vehicle that will be driven while on duty for Bronson is required. It is the employee's responsibility to submit auto insurance renewals before current coverage expires to their supervisor. It is also the employee's responsibility to update their auto insurance coverage with Bronson if a different vehicle will be driven to perform business on behalf of Bronson.
